บทคัดย่อ

การวิจัยนี้เป็นการศึกษาเชิงพรรณนาแบบแบบวิเคราะห์แนวโน้มย้อนหลัง โดยใช้ข้อมูลทุติยภูมิ จากผลการประเมินประสิทธิภาพหน่วยบริการโรงพยาบาลสังกัดสำนักงานปลัดกระทรวงสาธารณสุข เขตสุขภาพที่ 11 จำนวน 82 แห่ง ในปีงบประมาณ พ.ศ. 2566–2568 เพื่อวิเคราะห์ผลการประเมินการบริหารประสิทธิภาพ หลังสถานการณ์โควิด-19 และเสนอแนวทางพัฒนาประสิทธิภาพด้านการบริหารการเงินการคลังของหน่วยบริการ สถิติที่ใช้ในการวิจัย คือ ความถี่ ร้อยละ ค่าเฉลี่ย และส่วนเบี่ยงเบนมาตรฐาน ผลการศึกษาพบว่า

ประสิทธิภาพการบริหารมีแนวโน้มลดลงอย่างต่อเนื่อง โดยสัดส่วนหน่วยบริการที่ได้รับระดับคะแนนประเมิน TPS ดีมาก (Grade A) ในช่วงปีงบประมาณ 2567–2568 มีแนวโน้มลดลงเกือบครึ่งจากร้อยละ 26.8 เหลือร้อยละ 14.6 เช่นเดียวกับระดับดี (Grade B) และระดับพอใช้ (Grade C) ขณะที่ระดับต้องปรับปรุง (Grade D) และระดับไม่ผ่าน (Grade F) มีแนวโน้มเพิ่มขึ้น ตั้งแต่ปีงบประมาณ 2566-2568 สะท้อนถึงการถดถอยของประสิทธิภาพการบริหารจัดการหลังสถานการณ์การแพร่ระบาดโรคโควิด-19 ข้อเสนอแนะเชิงนโยบายเพื่อความยั่งยืน 1) การปฏิรูปกลไกจัดสรรงบประมาณกองทุนหลักประกันสุขภาพแห่งชาติ ให้สอดคล้องกับต้นทุนจริง 2) การพัฒนาระบบเฝ้าระวังและพยากรณ์ความเสี่ยงทางการเงินอัจฉริยะ และ 3) การยกระดับสมรรถนะบุคลากรด้านการเงินการคลัง เพื่อเสริมสร้างเสถียรภาพและความยั่งยืนของระบบสุขภาพในระดับเขตสุขภาพ
