บทคัดย่อ

การวิจัยแบบผสมผสานนี้มีวัตถุประสงค์เพื่อ 1) ประเมินระบบการดูแลระยะยาวด้านสาธารณสุขสำหรับผู้สูงอายุฯ และ 2) ศึกษาปัจจัยความสำเร็จ ปัญหา และอุปสรรคในการดำเนินงานระบบการดูแลระยะยาวด้านสาธารณสุขสำหรับผู้สูงอายุที่มีภาวะพึ่งพิงจังหวัดประจวบคีรีขันธ์ ประเมินผลตามรูปแบบ CIPP Model จำนวน 2 ระยะ คือ ระยะที่ 1 การวิจัยเชิงปริมาณ กลุ่มตัวอย่าง แบ่งเป็น 3 กลุ่ม คือ คณะอนุกรรมการ จำนวน 250 คน ผู้จัดการการดูแลผู้สูงอายุ จำนวน 55 คน และผู้สูงอายุที่มีภาวะพึ่งพิง จำนวน 288 คน เครื่องมือวิจัยเป็นแบบสอบถาม วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา ได้แก่ ร้อยละ ค่าเฉลี่ย และส่วนเบี่ยงเบนมาตรฐาน และระยะที่ 2 การวิจัยเชิงคุณภาพ กลุ่มตัวอย่าง จำนวน 36 คน เก็บรวบรวมข้อมูลด้วยการสนทนากลุ่ม วิเคราะห์ข้อมูลด้วยการวิเคราะห์เนื้อหา ผลการวิจัย พบว่า

1. ด้านบริบทมีนโยบาย “จังหวัดรักษ์สุขภาพ” ขับเคลื่อนผ่าน คณะกรรมการพัฒนาคุณภาพชีวิตระดับอำเภอ สำนักงานสาธารณสุขจังหวัด และสำนักงานหลักประกันสุขภาพแห่งชาติ ด้านปัจจัยนำเข้า คณะอนุกรรมการมีความรู้ระดับดี และการบริหารจัดการระดับมาก ด้านกระบวนการมีความเข้มแข็งด้านการเยี่ยมบ้านและการจัดทำแผนการดูแลรายบุคคลระดับดีเด่น ด้านผลผลิต ผู้สูงอายุมีความสามารถปฏิบัติกิจวัตรประจำวันดีขึ้น (ร้อยละ 23.30) และมีความพึงพอใจระดับมาก

2. ปัจจัยแห่งความสำเร็จที่สำคัญ คือ ภาวะผู้นำของผู้บริหารท้องถิ่น ด้านปัญหา พบว่า ระบบ การบันทึกข้อมูลซ้ำซ้อน

ผลการวิจัยสะท้อนว่าภาวะผู้นำของผู้บริหารเป็นปัจจัยสำคัญที่จะขับเคลื่อนระบบการดูแลระยะยาวด้านสาธารณสุขสำหรับผู้สูงอายุฯการเชื่อมโยงระบบสารสนเทศจะลดความซ้ำซ้อนที่ช่วยเพิ่มประสิทธิภาพการดำเนินงาน
