Development of discharge planning program with participation for stroke patients care in Wiangsa Crown prince Hospital, Surat Thani province

Background: After hospital discharge, stroke patients often face limitations in self-care and require ongoing reliance on caregivers and family members. Therefore, discharge planning should begin upon admission, continue throughout the hospital stay ongoing care at home.
Objective: It was aim to study discharge planning program with participation for stroke patients care and evaluate the outcomes of patient care in Wiangsa Crown prince Hospital, Surat Thani province.
Methods: This action research. Between October 2025 - February 2026. The sample group consisted of 25 staff members, 41 caregivers, and 41 patients. Research instrument used was a questionnaire, with an overall reliability coefficient of alpha = 0.88. Data were analyzed using descriptive statistics, 95% confidence interval and paired t-test.
Results: Development results 1) Plan: The hospital's existing approach was found to have limitations: lack of assessment caregiver's readiness. 2)Do: Shortcomings of the original process focus on information delivery rather than skill training, and personnel working in silos. Newly developed process focuses on enhancing personnel and teaching skills caregivers. 3)Check: By assessing personnel's practical skills and evaluating knowledge and potential of caregivers. 4)Act: Evaluate implementation of program. After development, found that overall level of personnel providing care to stroke patients increased significantly, from 72.0% to 92.0%, statistically significant increase (95%CI = 9.72-14.64, p<0.001). Caregivers showed a statistically significant increase in their overall knowledge of caring for stroke patients (95%CI =2.14-3.26, p =0.033) and overall level of patient care capabilities increased significantly (95%CI =1.57-1.76, p <0.001). Patient care outcomes showed that neurological symptoms showed a statistically significant reduction (p<0.001), shifting from severe impairment (43.9%) to mild-to-moderate impairment (53.7%).
Conclusion: This developing discharge planning program approach to stroke patient care ensure capabilities of caregivers and patients reduced severity of neurological symptoms.
