บทคัดย่อ

การทำข้อตกลงเกี่ยวกับการใช้ยา เป็นการทำข้อตกลงระหว่างบริษัทยากับผู้จ่ายเงินที่ช่วยให้ผู้ป่วยเข้าถึงยาใหม่ที่มีราคาแพงได้ ข้อตกลงนี้ แบ่งเป็น 2 ประเภท ได้แก่ ข้อตกลงทางการเงิน (financial-based agreement) และ ข้อตกลงตามประสิทธิผลของยา (performance-based agreement) สำหรับ financial-based agreement เป็นข้อตกลงทางด้านการเงินที่มีเป้าหมายเพื่อลดผลกระทบต่องบประมาณ ด้วยการควบคุมต้นทุนของยา ข้อตกลงประเภทนี้ถูกนำมาใช้อย่างแพร่หลาย เนื่องจากเป็นวิธีที่ง่ายและการบริหารจัดการไม่ซับซ้อน แต่เป็นวิธีที่ขาดความโปร่งใสเพราะรายละเอียดที่อยู่ในข้อตกลงเป็นความลับ อาจทำให้คู่แข่งบางรายเกิดผลประโยชน์ทับซ้อนจากรายละเอียดข้อตกลง สำหรับ performance-based agreement เป็นข้อตกลงค่าใช้จ่ายยาตามประสิทธิผลการรักษาของยาที่เกิดขึ้นจริง เพื่อจัดการกับความไม่แน่นอนเกี่ยวกับประสิทธิผลของยาใหม่ ข้อตกลงประเภทนี้เป็นวิธีในการแก้ปัญหาที่ดี แต่มีความซับซ้อนในการดำเนินการและมีค่าใช้จ่ายในการดำเนินการสูง อย่างไรก็ตาม ผู้ที่มีอำนาจตัดสินใจในระบบสุขภาพจะต้องพิจารณาอย่างรอบด้านในการเลือกใช้ข้อตกลงเกี่ยวกับการใช้ยาเพื่อให้การใช้งบประมาณเป็นไปอย่างมีประสิทธิภาพและผู้ป่วยเข้าถึงยาได้อย่างเท่าเทียม
