บทนำ : คุณภาพชีวิตของผู้ป่วยมะเร็งลำไส้ใหญ่และทวารหนักที่ได้รับยาเคมีบำบัดมักได้รับผลกระทบจากอาการข้างเคียงทางร่างกายและจิตใจ การประเมินความต้องการจำเป็นของคุณภาพชีวิตด้านสุขภาพ
จึงเป็นพื้นฐานสำคัญในการวางแผนการดูแลที่เหมาะสม
วัตถุประสงค์ : เพื่อประเมินความต้องการจำเป็น จัดลำดับความสำคัญ และวิเคราะห์ปัจจัยเชิงสาเหตุความ ต้องการจำเป็นของคุณภาพชีวิตด้านสุขภาพในผู้ป่วยมะเร็งลำไส้ใหญ่และทวารหนักที่ได้รับยาเคมีบำบัด
วิธีดำเนินการวิจัย : กลุ่มตัวอย่างการวิจัยเชิงพรรณนานี้ คือ ผู้ป่วยมะเร็งลำไส้และทวารหนักที่ได้รับยาเคมีบำบัด จำนวน 220 ราย โรงพยาบาลระดับตติยภูมิ กรุงเทพมหานคร โดยใช้แบบประเมินคุณภาพชีวิตด้านสุขภาพวิเคราะห์ความต้องการจำเป็น การจัดลำดับความสำคัญ และการวิเคราะห์ปัจจัยเชิงสาเหตุด้วยกระบวนการกลุ่มโดยใช้เทคนิคสมมตินัย ข้อมูลที่ได้จากกระบวนการกลุ่มนำมาวิเคราะห์เนื้อหาโดยประยุกต์ใช้ขั้นตอนของ Van Manen และสร้างแบบสรุปผลการวิเคราะห์ด้วยเทคนิคแผนภูมิก้างปลาตามแนวคิดของ Ishikawa
ผลการวิจัย : ความต้องการจำเป็นของคุณภาพชีวิตด้านสุขภาพจากการศึกษานี้ตามมิติ PNI modified อยู่ระดับปานกลางถึงสูง ได้แก่ อาการที่ส่งผลต่อสุขภาพ (0.34) รองลงมาคือ การทำหน้าที่ทางชีวภาพ (0.29) สภาวะการทำหน้าที่ของร่างกาย (0.26) และการรับรู้ภาวะสุขภาพ (0.22) ตามลำดับ ส่วนปัจจัยเชิงสาเหตุที่ส่งผลที่เกี่ยวข้องกับการตอบสนองต่อความต้องการจำเป็นของคุณภาพชีวิตด้านสุขภาพ ได้แก่ ข้อจำกัดของผู้ให้บริการสุขภาพ ระบบบริการสุขภาพ และครอบครัวผู้ป่วย
สรุป : ผู้ป่วยมะเร็งลำไส้ใหญ่และทวารหนักที่ได้รับเคมีบำบัดมีความต้องการจำเป็นด้านสุขภาพในหลายมิติการพัฒนารูปแบบการดูแลแบบองค์รวมที่ครอบคลุมทุกมิติสุขภาวะ การเสริมทักษะทีมสุขภาพ และการ สนับสนุนครอบครัว จะช่วยส่งเสริมคุณภาพชีวิตด้านสุขภาพของผู้ป่วยอย่างมีประสิทธิภาพได้
