การศึกษาสภาพปัจจุบัน ปัญหา และความต้องการในการใช้เครื่องมือช่วยวินิจฉัย อายุกระดูกและติดตามการเจริญเติบโตในเด็กไทย

บทนำ : การประเมินอายุกระดูกมีความสำคัญต่อการวินิจฉัยและติดตามการเจริญเติบโตของเด็ก แม้ว่าวิธีกรูลิชแอนด์ไพล์ (GP) จะใช้กันแพร่หลาย แต่ก็มีข้อจำกัดด้านความแม่นยำและความแปรปรวน ในขณะที่ต่างประเทศใช้ปัญญาประดิษฐ์ (AI) เพื่อเพิ่มประสิทธิภาพการประเมิน ประเทศไทยยังขาดข้อมูลด้านสภาพปัจจุบัน ปัญหา และความต้องการในการนำ AI มาใช้ในบริบทนี้
วัตถุประสงค์ : เพื่อศึกษาสภาพปัจจุบัน ปัญหา และความต้องการในการใช้เครื่องมือช่วยวินิจฉัยอายุกระดูกและติดตามการเจริญเติบโต
วิธีดำเนินการวิจัย: เป็นการวิจัยเชิงคุณภาพ โดยการศึกษาเอกสารและสัมภาษณ์เชิงลึกในกลุ่มผู้ใช้งาน 2 กลุ่ม ได้แก่ แพทย์ 6 คน (กุมารแพทย์ 3 คน, แพทย์ด้านรังสี 3 คน) และกลุ่มผู้ปกครองที่เกี่ยวข้องอีก 6 คน ณ โรงพยาบาลศรีนครินทร์ มหาวิทยาลัยขอนแก่น จากนั้นนำข้อมูลทั้งหมดมาวิเคราะห์เชิงเนื้อหา
ผลการศึกษา : สภาพปัจจุบัน: แพทย์ผู้ให้ข้อมูลทั้งหมดใช้วิธี GP เนื่องจากสะดวกและรวดเร็ว ปัญหา: วิธี GP มีข้อจำกัดด้านความแปรปรวนระหว่างผู้ประเมินเพราะต้องอาศัยประสบการณ์ของแพทย์ในบางช่วงอายุ ขณะที่ AI ของต่างประเทศมีค่าใช้จ่ายสูงและยังไม่แพร่หลายในไทย อีกทั้งผู้ปกครองยังขาดความรู้ในการสังเกตพัฒนาการเข้าสู่วัยรุ่นก่อนวัย ทำให้การตรวจพบความผิดปกติล่าช้า ความต้องการ: แพทย์เห็นความจำเป็นในการพัฒนา AI ที่เหมาะสมกับบริบทเด็กไทย เพื่อเพิ่มความแม่นยำและลดความแปรปรวนในการประเมิน และเสนอให้พัฒนาเป็นแอปพลิเคชันสำหรับแพทย์และผู้ปกครอง ที่สามารถประเมินอายุกระดูก คาดการณ์ความสูงสุดท้าย และติดตามการเจริญเติบโตทุติยภูมิได้อย่างต่อเนื่อง ซึ่งสอดคล้องกับความต้องการของผู้ปกครอง
สรุป : ปัจจุบันแพทย์นิยมใช้วิธี GP แต่ยังมีความซับซ้อนและความแปรปรวนสูง แนวทางที่มีศักยภาพและตอบโจทย์ความต้องการของทั้งแพทย์และผู้ปกครอง คือ การพัฒนา AI เพื่อประเมินอายุกระดูก ควบคู่กับการพัฒนาแอปพลิเคชันที่สะดวกและเหมาะสม เพื่อรองรับการติดตามการเจริญเติบโตได้อย่างต่อเนื่องในบริบทของเด็กไทย
