บทนำ: การบาดเจ็บที่สมองเป็นปัญหาสาธารณสุขสำคัญที่ก่อให้เกิดการเสียชีวิต ความพิการ คุณภาพชีวิตลดลง และค่าใช้จ่ายในการรักษาสูง แม้ได้รับการผ่าตัด ผู้ป่วยจำนวนมากยังคงเผชิญภาวะแทรกซ้อนและผลลัพธ์ทางคลินิกที่ดีและไม่ดี การทำความเข้าใจปัจจัยที่มีผลต่อผลลัพธ์เหล่านี้จึงมีความสำคัญต่อการพัฒนาการดูแลรักษาที่มีประสิทธิภาพยิ่งขึ้น
วัตถุประสงค์: เพื่อศึกษาปัจจัยที่มีความสัมพันธ์กับผลลัพธ์ทางคลินิกภายหลังผ่าตัดในผู้ป่วย Traumatic brain injury ที่เข้ารับการรักษาในโรงพยาบาลกระบี่
วิธีดำเนินการวิจัย: การวิจัยนี้เป็นการวิจัยย้อนหลัง (Retrospective cohort study) โดยเก็บข้อมูลจากเวชระเบียนผู้ป่วย โรงพยาบาลกระบี่ กลุ่มตัวอย่างคือ ข้อมูลเวชระเบียนผู้ป่วยบาดเจ็บที่สมองที่ได้รับการผ่าตัดในโรงพยาบาลกระบี่ ตั้งแต่ปี พ.ศ. 2563 ถึง 2567 จำนวน 308 ราย เครื่องมือที่ใช้ในการเก็บรวบรวมข้อมูล ประกอบด้วย 1) แบบบันทึกปัจจัยส่วนบุคคลและภาวะสุขภาพ และ 2) แบบบันทึกข้อมูลปัจจัยด้านลักษณะการบาดเจ็บและการรักษา วิเคราะห์ข้อมูลส่วนบุคคลโดยใช้สถิติเชิงพรรณนา ได้แก่ จำนวน ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน

ผลการวิจัย: ปัจจัยที่มีความสัมพันธ์อย่างมีนัยสำคัญกับผลลัพธ์หลังการผ่าตัดในผู้ป่วยบาดเจ็บทางสมอง ได้แก่ ระดับความรู้สึกตัวเมื่อเข้ารับการรักษาครั้งแรก (rₛ = -.666) ระยะเวลาตั้งแต่มาถึงโรงพยาบาลจนถึงการผ่าตัด (rₛ = -0.605) การเคลื่อนที่ของแนวกึ่งกลางสมอง (rₛ = -0.564) ภาวะความดันโลหิตต่ำ (rₛ = -0.597) ขนาดและการตอบสนองของรูม่านตา (rₛ = -0.637) ชนิดของการผ่าตัด (rₛ = -0.546) และภาวะแทรกซ้อนหลังผ่าตัด (rₛ = -0.622) มีความสัมพันธ์เชิงลบในระดับสูงอย่างมีนัยสำคัญทางสถิติที่ระดับ p < .001
สรุป: ปัจจัยที่มีความสัมพันธ์อย่างมีนัยสำคัญกับผลลัพธ์ทางคลินิกหลังผ่าตัดในผู้ป่วย TBI ได้แก่ ระดับความรู้สึกตัว ระยะเวลาสู่การรักษา ความรุนแรงของอาการทางระบบประสาท การผ่าตัดและภาวะแทรกซ้อน โดยเฉพาะตัวแปรที่สะท้อนความรุนแรงและความรวดเร็วในการดูแลรักษามีบทบาทสำคัญต่อผลลัพธ์ของผู้ป่วย
