ผลการออกกำลังกายต่อมวลกล้ามเนื้อและสมรรถภาพทางกายในผู้สูงอายุที่มีภาวะมวลกล้ามเนื้อน้อย: การทบทวนวรรณกรรมอย่างเป็นระบบ

บทคัดย่อ

บทนำ : ภาวะมวลกล้ามเนื้อน้อย (Sarcopenia) เป็นปัญหาสำคัญในผู้สูงอายุทั่วโลก เนื่องจากส่งผลต่อการลดลงของสมรรถภาพทางกาย ความสามารถในการดำเนินชีวิต การป้องกันและรักษาภาวะดังกล่าวจึงมีความสำคัญอย่างยิ่ง ปัจจุบัน การออกกำลังกายถือเป็นวิธีการรักษาที่มีประสิทธิภาพสูงสุด อย่างไรก็ตาม ยังมีข้อจำกัดในด้านประเภท ความถี่ และระดับความหนักของการออกกำลังกายที่เหมาะสมต่อการเปลี่ยนแปลงของตัวแปรต่าง ๆ ที่เกี่ยวข้อง

วัตถุประสงค์ : เพื่อทบทวนและสังเคราะห์หลักฐานเชิงประจักษ์เกี่ยวกับประสิทธิผลของการออกกำลังกายต่อมวลกล้ามเนื้อ และสมรรถภาพทางกายของผู้สูงอายุที่มีภาวะมวลกล้ามเนื้อน้อย

วิธีดำเนินการวิจัย : การศึกษานี้เป็นการทบทวนวรรณกรรมอย่างเป็นระบบ โดยทำการสืบค้นงานวิจัยแบบสุ่มและมีกลุ่มควบคุม (Randomized Controlled Trials) จากฐานข้อมูล
ที่น่าเชื่อถือ ได้แก่ PubMed ScienceDirect Cochrane Library CINAHL Embase และ Scopus งานวิจัยที่ผ่านการคัดเลือกต้องเป็นการศึกษาที่ดำเนินการในกลุ่มผู้สูงอายุที่มีภาวะมวลกล้ามเนื้อน้อย โดยผ่านกระบวนการคัดกรองและประเมินคุณภาพของงานวิจัยโดยนักวิจัย 2 คนอย่างเป็นอิสระ พร้อมทั้งใช้ PEDro scale ในการประเมินคุณภาพของงานวิจัยแต่ละฉบับ

ผลการศึกษา : จากการสืบค้นพบงานวิจัยทั้งหมด 336 เรื่อง โดยมีจำนวน 16 เรื่องที่ผ่านเกณฑ์การคัดเลือก รวมจำนวนผู้เข้าร่วมทั้งหมด 820 คน การวิเคราะห์ข้อมูลพบว่า การออกกำลังกายทุกรูปแบบสามารถส่งเสริมการเพิ่มมวลกล้ามเนื้อ ความแข็งแรง และสมรรถภาพทางกายของผู้สูงอายุที่มีภาวะมวลกล้ามเนื้อน้อยได้ โดยเฉพาะอย่างยิ่ง การออกกำลังกายแบบมีแรงต้าน (Resistance Exercise)

สรุป : การออกกำลังกาย โดยเฉพาะการฝึกแบบมีแรงต้าน เป็นวิธีที่มีประสิทธิภาพในการเพิ่มมวลกล้ามเนื้อ และสมรรถภาพทางกายของผู้สูงอายุที่มีภาวะมวลกล้ามเนื้อน้อย โดยแนะนำให้ทำอย่างน้อย 3–4 ครั้งต่อสัปดาห์ เป็นเวลาไม่น้อยกว่า 12 สัปดาห์ และมีระดับความหนักอยู่ที่
ร้อยละ 40–60 ของน้ำหนักสูงสุดที่สามารถยกได้ 1 ครั้ง
