Bilateral ovarian coexistence of mature cystic teratomas and endometriomas: a unique case report

บทคัดย่อ

เนื้องอกถุงน้ำรังไข่ชนิดแมทชัวร์เทอราโตมา (mature cystic teratoma, MCT) และถุงน้ำรังไข่ชนิดเยื่อบุโพรงมดลูกเจริญผิดที่ (endometrioma) จัดเป็นโรคของรังไข่ชนิดไม่ร้ายแรงที่พบได้บ่อยในสตรีวัยเจริญพันธุ์ แม้ว่าจะตรวจพบได้ทั่วไป แต่การเกิดโรคร่วมในรังไข่ข้างเดียวกันนั้นพบได้ยาก และยังไม่เคยมีรายงานการเกิดร่วมกันในรังไข่ทั้งสองข้างมาก่อน รายงานฉบับนี้นำเสนอผู้ป่วยสตรีอายุ 33 ปี ที่ไม่เคยตั้งครรภ์ มาด้วยอาการก้อนใหญ่ขึ้นในอุ้งเชิงกราน พบสารบ่งชี้มะเร็งในเลือดมีค่าสูง ได้แก่ CA125 และ CA19-9 ผลการตรวจทางรังสีวินิจฉัยพบก้อนที่รังไข่ทั้งสองข้าง ซึ่งมีลักษณะเข้าได้กับ MCT โดยสงสัยว่าอาจมีเนื้องอกต่างชนิดพบร่วมกันด้วยที่รังไข่ข้างซ้าย ผู้ป่วยจึงได้รับการผ่าตัดเอาปีกมดลูกและรังไข่ออกทั้งสองข้าง ผลการตรวจทางพยาธิวิทยายืนยันการพบทั้ง MCT และ endometrioma ในรังไข่ทั้งสองข้าง นอกจากนี้ยังพบถุงน้ำฟอลลิเคิล (cystic follicle) ในรังไข่ทั้งสองข้างด้วย จากการทบทวนวรรณกรรม การศึกษานี้ถือเป็นรายงานผู้ป่วยรายแรกที่พบการเกิดร่วมกันของ MCT และ endometrioma ในรังไข่ทั้งสองข้าง ที่นำเสนอข้อมูลทั้งจากการตรวจด้วยตาเปล่า และการตรวจด้วยกล้องจุลทรรศน์
