ปัจจัยที่มีผลต่อการอุดตันซ้ำหลังการผ่าตัดแก้ไขท่อน้ำตาอุดตัน
บทคัดย่อ
วัตถุประสงค์: เพื่อศึกษาปัจจัยที่มีผลต่อความล้มเหลวของการผ่าตัดรักษาท่อน้ำตาอุดตัน โดยการส่องกล้องทางจมูก
ระเบียบวิธีวิจัย: การศึกษาแบบ retrospective cohort
วิธีการ: รวบรวมข้อมูลย้อนหลังในผู้ป่วยที่ได้รับการผ่าตัดท่อน้ำตาอุดตันโดยการส่องกล้องทางจมูกโรงพยาบาลนครพิงค์
ระหว่าง กรกฎาคม 2550 ถึง มิถุนายน 2560 ติดตามผลอย่างน้อย 6 เดือน ผลลัพธ์คือการรักษาล้มเหลวหมายถึง น้ำตาไหล
เหมือนเดิม ล้างท่อน้ำตาไม่ไหลลงคอ
ผลการรักษา: ผู้ป่วยจำนวน 102 ราย 137 ตา เป็นหญิงร้อยละ 85.36 อายุเฉลี่ย 58.04±15.66 ปี ระยะเวลามีอาการเฉลี่ย
2 ปี ประวัติการเกิดถุงน้ำตาอักเสบร้อยละ 31.50 พยาธิสภาพทางจมูกและเนื้อเยื่อรอบทางเดินน้ำตาร่วมร้อยละ 23.40 ปัจจัย
ที่มีผลต่อความล้มเหลวในการผ่าตัดได้แก่ การมีถุงน้ำตาอักเสบ (Adj. RR 2.60 (95%CI 1.13-5.99, p=0.024)) และการมี
พยาธิสภาพทางจมูกและเนื้อเยื่อรอบทางเดินน้ำตา (Adj. RR 2.17 (95%CI 1.21-3.87, p=0.009))
สรุป: แพทย์ผู้ทำการผ่าตัดและผู้ป่วยต้องวางแผนการรักษาร่วมกัน โดยเฉพาะผู้ป่วยที่มีประวัติการอักเสบของถุงน้ำตาควร
แนะนำให้ผ่าตัดเร็วขึ้น
