ภาวะจอตาบวมน้ำที่พบร่วมกับเนื้องอกของเยื่อหุ้มสมอง : รายงานผู้ป่วย

บทคัดย่อ

วัตถุประสงค์:  เพื่อรายงานกรณีผู้ป่วยที่มีเนื้องอกของเยื่อหุ้มสมองชั้นเหนือเซลล่า (Suprasellarmeningioma)ซึ่งมีลักษณทางคลินิก คล้ายกับภาวะจอตาบวมจากน้ำรั่ว (Central serouschorioretinopathy)และแสดงให้เห็นถึงความสำคัญของการตรวจหาความผิดปกติในสมอง เมื่อการมองเห็นไม่ดีขึ้นภายหลังการรักษาโรคของจอตา

รายงานผู้ป่วย:  ผู้ป่วยหญิงอายุ 41 ปีมาพบแพทย์ด้วยอาการตาขวามัวมา 3 เดือน อาการตามัวค่อยๆเป็นมากขึ้น และมีการสูญเสียการมองเห็นส่วนกลางโดยไม่มีอาการปวดตาตรวจร่างกายทางจักษุวิทยาและการตรวจโดยการถ่ายภาพตัดขวางจอตา (OCT) พบว่ามีน้ำใต้ต่อจอตาบริเวณจุดรับภาพชัดของตาขวาซึ่งเข้าได้กับภาวะจอตาบวมจากน้ำรั่ว(CSCR)ผลการตรวจจอตาด้วยการฉีดสารเรืองแสงฟลูออเรสซีนและอินโดไซยานีนกรีน (FFA ICGA) แสดงให้เห็นจุดรั่วซึมของน้ำนอกบริเวณจุดรับภาพชัด ผู้ป่วยจึงได้รับการรักษา โดยการใช้เลเซอร์โฟโตโคแอกกูเลชั่น ภายหลังการรักษาผู้ป่วยยังคงมีการมองเห็นแย่ลงผลการตรวจลานสายตาพบลักษณะภาวะลานสายตาบกพร่องครึ่งซีกด้านนอกทั้งสองข้าง (Bitemporalhemianopia) ทำให้สงสัยรอยโรคในเส้นทางการมองเห็น (visual pathway) ผู้ป่วยจึงได้รับการตรวจคลื่นแม่เหล็กไฟฟ้า(MRI) บริเวณสมองและเบ้าตา พบก้อนเนื้องอกของเยื่อหุ้มสมองบริเวณเหนือเซลล่าซึ่งกดเบียดเส้นประสาทตา สุดท้ายผู้ป่วยได้รับการผ่าตัด และผลการตรวจชิ้นเนื้อเป็นเนื้องอกเยื่อหุ้มสมอง
สรุป:  ประเด็นสำคัญของกรณีศึกษาผู้ป่วยรายนี้คือโรคเนื้องอกในสมองอาจมีอาการแสดงคล้ายหรืออาจเกิดร่วมกันกับความผิดปกติทางจอตาซึ่งทำให้การวินิจฉัยทำได้ยากขึ้นหรือวินิจฉัยได้ล่าช้าในผู้ป่วยที่ยังคงมีการสูญเสียการมองเห็นแม้ว่าภาวะจอตาบวมจากน้ำรั่วดีขึ้นแล้ว ควรนึกถึงโรคหรือภาวะอื่น การตรวจภาพทางระบบประสาทโดยเฉพาะการทำ MRI สมองและเบ้าตาจะช่วยให้สามารถตรวจพบรอยโรคในสมองและนำไปสู่การรักษาที่รวดเร็วยิ่งขึ้น
