ผลการรักษาของการผ่าตัด Pelvic Exenteration ในมะเร็งลำไส้ใหญ่และระยะลุกลามเฉพาะที่

ภูมิหลัง: การผ่าตัด Pelvic Exenteration (PE) เป็นการผ่าตัดขนาดใหญ่และมีความซับซ้อนสูง ใช้รักษาผู้ป่วยมะเร็งลำไส้ใหญ่และไส้ตรงระยะลุกลามเฉพาะที่ ซึ่งไม่สามารถตัดออกได้ด้วยการผ่าตัดมาตรฐาน เป้าหมายสำคัญคือการได้ขอบเขตการตัดที่ปลอดจากเซลล์มะเร็ง (R0 resection) ข้อมูลผลการรักษาจากโรงพยาบาลระดับทั่วไปในประเทศไทยยังมีจำกัด วัตถุประสงค์: เพื่อประเมินผลการรักษาและภาวะแทรกซ้อนของการผ่าตัด pelvic exenteration ในผู้ป่วยมะเร็งลำไส้ใหญ่และไส้ตรงระยะลุกลามเฉพาะที่ ณ โรงพยาบาลนพรัตนราชธานี วิธีการ: การศึกษาย้อนหลังเชิงพรรณนา[SK1] ในผู้ป่วยมะเร็งลำไส้ใหญ่และไส้ตรงระยะลุกลามเฉพาะที่ ที่ได้รับการผ่าตัด pelvic exenteration ระหว่างเดือนพฤศจิกายน พ.ศ. 2564 ถึงมกราคม พ.ศ. 2568 วิเคราะห์ข้อมูลพื้นฐาน รายละเอียดการผ่าตัด ผลพยาธิวิทยา และผลลัพธ์หลังผ่าตัด โดยใช้สถิติเชิงพรรณนา ผล: ผู้ป่วยจำนวน 9 ราย เป็นเพศหญิง 6 ราย (ร้อยละ 66.7) อายุเฉลี่ย 60.8 ± 7.1 ปี ได้รับการผ่าตัดแบบ posterior pelvic exenteration (PPE) 6 ราย และ total pelvic exenteration (TPE) 3 ราย ระยะเวลาผ่าตัดเฉลี่ย 474 ± 91 นาที และปริมาณการเสียเลือดเฉลี่ย 1,178 ± 554 มิลลิลิตร ได้ขอบเขตการตัดปลอดเซลล์มะเร็ง (R0 resection) 8 ราย (ร้อยละ 88.9) ภาวะแทรกซ้อนหลังผ่าตัดพบเฉพาะแผลผ่าตัดติดเชื้อ 2 ราย (ร้อยละ 22.2) ไม่พบการเสียชีวิตภายใน 30 วัน ระยะเวลานอนโรงพยาบาลมัธยฐาน 13 วัน (IQR 11–15) ในระยะติดตามผล พบอัตราการรอดชีวิตรวมร้อยละ 77.8 และอัตราการรอดชีวิตปลอดโรคในผู้ป่วยที่ยังมีชีวิตอยู่ร้อยละ 55.6 สรุป: การผ่าตัด pelvic exenteration สามารถทำได้อย่างปลอดภัยในโรงพยาบาลระดับทั่วไป หากมีการคัดเลือกผู้ป่วยที่เหมาะสมและการดูแลแบบสหสาขาวิชาชีพ โดยให้ผลลัพธ์ด้านการผ่าตัดและการควบคุมโรคในระยะสั้นที่น่าพอใจ
