บทคัดย่อ
บทนำ: Solitary fibrous tumor (SFT) เป็นเนื้องอกของเนื้อเยื่อเกี่ยวพันกลุ่มไฟโบรบลาสต์ที่พบได้น้อยมาก มีอณูพันธุศาสตร์ที่จำ เฉพาะต่อ NAB2-STAT6 gene fusion ที่ตำแหน่ง 12q13 พบเนื้องอกชนิดนี้ได้หลายตำแหน่งทั่วร่างกาย SFT ที่กระเพาะปัสสาวะพบได้น้อยมาก โดยผู้แต่งรายงานผู้ป่วยครั้งนี้จำ นวน 27 ราย แบ่งเป็นผู้ป่วยกรณีศึกษาที่มารับการรักษาจำนวน 1 รายและผู้ป่วยกรณีศึกษาจากการทบทวนวรรณกรรมที่เกี่ยวข้องจำ นวน 26 ราย
รายงานผู้ป่วย: ผู้ป่วยชายไทย อายุ 45 ปี มาพบแพทย์ด้วยอาการปวดท้องน้อยมากขึ้น ตรวจร่างกายพบก้อนในช่องท้องด้านซ้ายล่าง เอกซเรย์คอมพิวเตอร์พบเนื้องอกขนาด 5.6x5.1 เซนติเมตร บริเวณผนังด้านในซ้ายของกระเพาะปัสสาวะ ผู้ป่วยได้รับการผ่าตัดกระเพาะปัสสาวะพบก้อนเนื้องอกขนาด 4x2.5x2.2 เซนติเมตร ผลจุลพยาธิวิทยาร่วมกับการย้อมอิมมูโนฮีสโตเคมีสรุปผลการวินิจฉัยเป็น Solitary fibrous tumor ของกระเพาะปัสสาวะ ไม่พบการกลับเป็นซ้ำ หรือแพร่กระจายหลังจากตรวจติดตามเป็นเวลา 4 เดือน
สรุป: SFT มีการอาการทางคลินิกหลากหลายและไม่เฉพาะเจาะจง วินิจฉัยจากผลจุลพยาธิวิทยาของเนื้องอกร่วมกับการย้อมอิมมูโนฮีสโตเคมี รักษาโดยการผ่าตัดเป็นหลักร่วมกับเคมีบำ บัดและรังสีรักษาเมื่อมีข้อบ่งชี้ ปัจจัยที่ส่งผลต่อการพยากรณ์โรค คือ อายุ ขนาดเนื้องอก บริเวณเนื้อตาย Mitotic figure และ Surgical margin status
