ปัจจัยก่อนการบาดเจ็บที่สัมพันธ์กับคะแนน Barthel Index ก่อนจำหน่ายใน ผู้สูงอายุหลังผ่าตัดกระดูกสะโพกหัก: การศึกษาแบบ Retrospective Cohort

วัตถุประสงค์: กระดูกสะโพกหักในผู้สูงอายุเป็นปัญหาสาธารณสุขที่สำคัญ และเป็นสาเหตุสำคัญของการสูญเสียความสามารถในการพึ่งพาตนเอง การศึกษานี้มีวัตถุประสงค์เพื่อศึกษาปัจจัยก่อนการบาดเจ็บที่มีความสัมพันธ์กับความสามารถในการดำเนินกิจวัตรประจำวันขั้นพื้นฐานก่อนจำหน่ายออกจากโรงพยาบาลในผู้ป่วยสูงอายุที่ได้รับการผ่าตัดรักษากระดูกสะโพกหักจากภยันตรายไม่รุนแรง
วิธีการศึกษา: การศึกษานี้เป็นการวิจัยเชิงวิเคราะห์แบบย้อนหลัง (retrospective cohort study) ศึกษาผู้ป่วยอายุ 60 ปีขึ้นไป ที่ได้รับการผ่าตัดรักษากระดูกสะโพกหักจากภยันตรายชนิดไม่รุนแรง (Fragility fracture) ระหว่างเดือนมกราคม พ.ศ. 2567 ถึงเดือนธันวาคม พ.ศ. 2568 จำนวน 361 ราย วิเคราะห์ความสัมพันธ์ระหว่างปัจจัยก่อนการบาดเจ็บกับคะแนน Barthel Index ก่อนจำหน่ายออกจากโรงพยาบาล โดยใช้สถิติเชิงพรรณนา การวิเคราะห์เชิงเดี่ยว และการวิเคราะห์ถดถอยเชิงเส้นพหุคูณ (multivariable linear regression) เพื่อระบุปัจจัยทำนายอิสระของคะแนน Barthel Index (BI) ก่อนจำหน่าย
ผลการศึกษา: กลุ่มตัวอย่างส่วนใหญ่เป็นเพศหญิง อายุเฉลี่ย 77.43 ปี คะแนน BI ก่อนจำหน่าย (51.55 ± 19.87) ลดลงอย่างมีนัยสำคัญเมื่อเทียบกับก่อนการบาดเจ็บ (91.33 ± 18.69; p < 0.001) การวิเคราะห์เชิงเดี่ยวพบว่า อายุ คะแนน BI ก่อนการบาดเจ็บ ภาวะสมองเสื่อมหรือความบกพร่องด้านสติปัญญา และระดับ 25-hydroxyvitamin D มีความสัมพันธ์กับคะแนน BI ก่อนจำหน่าย (p < 0.05) ในการวิเคราะห์พหุตัวแปร พบว่า อายุที่มากขึ้น (B = −0.42; 95% CI −0.61 ถึง −0.23; p < 0.001) และคะแนน BI ก่อนการบาดเจ็บ (B = 0.55; 95% CI 0.46–0.64; p < 0.001) เป็นปัจจัยทำนายอิสระที่มีความสัมพันธ์กับคะแนน BI ก่อนจำหน่าย ขณะที่โรคร่วมและระดับ 25-hydroxyvitamin D ไม่พบความสัมพันธ์อย่างอิสระในแบบจำลองสุดท้าย แบบจำลองสามารถอธิบายความแปรปรวนของผลลัพธ์ได้ร้อยละ 31.7 (Adjusted R² = 0.317)
