การศึกษาเปรียบเทียบการดำเนินของไตในผู้ป่วยโรคไตเรื้อรังระยะที่ 3–4 ที่มีภาวะกรดยูริกในเลือดสูงชนิดมีอาการและไม่มีอาการในโรงพยาบาลอุตรดิตถ์

บทคัดย่อ
ความเป็นมา: โรคไตเรื้อรังเป็นปัญหาสาธารณสุขสำคัญทั่วโลก พบมากกว่าร้อยละ 10 ของประชากร หากไม่ได้รับการดูแลที่เหมาะสม อาจดำเนินไปสู่ภาวะไตวายเรื้อรังระยะสุดท้ายซึ่งต้องบำบัดทดแทนไต และภาวะกรดยูริกในเลือดสูงเป็นภาวะที่พบได้บ่อยและสัมพันธ์กับโรคไม่ติดต่อเรื้อรังหลายชนิดรวมถึงโรคไตเรื้อรัง โดยชนิดมีอาการส่งผลต่อการดำเนินของไต ส่วนชนิดไม่มีอาการ ในปัจจุบันพบว่ายังคงมีข้อถกเถียงว่าส่งผลต่อการดำเนินของโรคไตเรื้อรังหรือไม่ ดังนั้นการศึกษาเพื่อหาข้อสรุปว่าภาวะกรดยูริกในเลือดสูง ที่ส่งผลให้เป็นโรคไตชนิดไม่มีอาการส่งผลต่อการดำเนินของโรคไตเรื้อรังหรือไม่ จึงยังเป็นคำถามที่ต้องการคำตอบ เพื่อนำไปสู่การพัฒนาแนวทางการรักษาที่มีประสิทธิผลสำหรับผู้ป่วยกลุ่มนี้ต่อไป
วัตถุประสงค์: เพื่อเปรียบเทียบการดำเนินของไตในผู้ป่วยโรคไตเรื้อรังที่มีภาวะกรดยูริกในเลือดสูงชนิดมีอาการและไม่มีอาการ
วิธีการศึกษา: ศึกษาผู้ป่วยที่มีภาวะกรดยูริกในเลือดสูง ตามรหัสข้อมูล ICD-10 ได้แก่ M1099 และ E790 ระหว่างวันที่ 1 มกราคม พ.ศ. 2563 ถึง 1 กันยายน พ.ศ. 2567 จำนวน 2,515 ราย โดยคัดเลือกแบบ Fixed Sample Sequential Sampling จำนวน 622 ราย ซึ่งเป็นผู้ป่วยที่ได้รับการวินิจฉัยเป็นโรคไตเรื้อรังระยะที่ 3-4 ได้จำนวน 200 ราย นำมาเปรียบเทียบปัจจัยเสี่ยงทางคลินิกที่มีผลต่อการดำเนินของไต จากนั้นวิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณา และสถิติเชิงวิเคราะห์
ผลการศึกษา: การศึกษานี้ทำในผู้ป่วยโรคไตเรื้อรังระยะที่ 3–4 ที่มีภาวะกรดยูริกในเลือดสูง จำนวน 200 ราย แบ่งเป็นชนิดมีอาการ 107 ราย และไม่มีอาการ 93 ราย พบว่าลักษณะพื้นฐานของทั้งสองกลุ่มไม่แตกต่างกัน นั่นคือโดยส่วนใหญ่ของผู้ป่วยในทั้งสองกลุ่มเป็นเพศชาย (p=0.861) มีอายุเฉลี่ย 73 ปี (p=0.344) มีระดับกรดยูริกในเลือดเฉลี่ย 8.5 mg/dL (p=0.802) ยกเว้นการใช้ยา colchicine ที่พบมากกว่าในกลุ่มมีอาการ (ร้อยละ 90.65 เทียบกับร้อยละ 35.48, p<0.001) การวิเคราะห์พบว่าระดับกรดยูริกที่สูงสัมพันธ์กับการลดลงของค่า eGFR อย่างมีนัยสำคัญ (β = –1.67, p =0.001) ขณะที่เพศชาย (β =3.99, p =0.030) ภาวะไขมันในเลือดสูง (β =6.67, p =0.044) และการใช้ยา RAAS inhibitors (β =5.07, p =0.003) มีความสัมพันธ์กับค่า eGFR ที่สูงขึ้น เมื่อปรับปัจจัยร่วม ได้แก่ โรคประจำตัวและยาที่เกี่ยวข้องทั้งหมดแล้ว ไม่พบความแตกต่างของอัตราการลดลงของค่า eGFR ระหว่าง

สรุป: แม้ว่าระดับกรดยูริกในเลือดที่สูงจะสัมพันธ์กับการลดลงของค่า eGFR อย่างมีนัยสำคัญทางสถิติ แต่เมื่อเปรียบเทียบระหว่างผู้ป่วยที่มีภาวะกรดยูริกในเลือดสูงชนิดมีอาการและชนิดไม่มีอาการ พบว่าอัตราการลดลงของค่า eGFR ไม่แตกต่างกัน (p=0.162)
