การแก้ไขตำแหน่งฟันเขี้ยวบนกับฟันตัดข้างบนที่สลับตำแหน่งกันโดยใช้เครื่องมือทันตกรรมจัดฟันชนิดติดแน่น: รายงานผู้ป่วย

บทคัดย่อ

รายงานผู้ป่วยนี้นำเสนอการแก้ไขฟันเขี้ยวแท้บนซ้ายขึ้นสูงผิดตำแหน่งที่พบได้ไม่บ่อยนักของเด็กหญิงไทยอายุ 12 ปีที่ถูกส่งต่อมาปรึกษาทันตแพทย์จัดฟันพร้อมกับผู้ปกครอง การตรวจภายนอกช่องปากพบว่าผู้ป่วยมีลักษณะใบหน้าตรงสมมาตรซ้าย-ขวา และมีสัดส่วนในแนวดิ่งปกติ, ลักษณะใบหน้าด้านข้างเป็นชนิดโค้งนูนเล็กน้อย (slightly convex profile) ตำแหน่งริมฝีปากบน-ล่างปกติ การตรวจภายในช่องปากพบมีฟันเขี้ยวน้ำนมบนซ้าย (ซี่ 63) ยังอยู่ในช่องปาก ในขณะที่ฟันเขี้ยวแท้บนซ้าย (ซี่ 23) งอกในระดับเหนือขอบเหงือกด้านบนสุดของฟันตัดข้างแท้บนซ้าย (ซี่ 22), มีระยะสบเหลื่อมฟันหน้าแนวราบ (overjet) 3 มิลลิเมตร, ระยะสบเหลื่อมฟันหน้าแนวดิ่ง (overbite) 4 มิลลิเมตร, พบมีฟันหน้าล่างซ้อนเกเล็กน้อย, เส้นกึ่งกลางฟันหน้าบนเอียงไปด้านซ้าย 2 มิลลิเมตร และการสบฟันกรามทั้งสองข้างและฟันเขี้ยวขวาเป็นชนิดความสัมพันธ์ชนิดที่ 1 (Class Irelationships) แต่การสบฟันเขี้ยวซ้ายเป็นชนิดความสัมพันธ์ชนิดที่ 2 (Class II relationships) แผนการรักษาคือการถอนฟันเขี้ยวน้ำนมบนซ้าย (ซี่ 63) และใช้เครื่องมือทันตกรรมจัดฟันชนิดติดแน่นเคลื่อนฟันเขี้ยวแท้บนซ้าย (ซี่ 23) ให้อยู่ในตำแหน่งที่ถูกต้องร่วมกับการแก้ไขปัญหาการสบฟันผิดปกติอื่นๆ ภายหลังการรักษาพบว่าการเรียงตัวของฟัน, การสบฟันและอวัยวะปริทันต์อยู่ในช่วงค่าปกติ และจากการติดตามภายหลังการรักษาพบว่าการสบฟันมีเสถียรภาพ (Stability) ที่ดี
