This study aimed to identify clinical manifestations and risk factors for Pneumonia in Diabetes Mellitus with COVID-19. Methods. This was a retrospective cohort study that collected data from the Medical records of all patients who had Diabetes Mellitus with COVID-19. RT-PCR or ATK confirmed COVID-19 from nasopharyngeal specimens.           All patients were treated in Huaiyot Hospital, Trang Province between March 2021 and September 2022. Demographic data, severity of illness, and possible factors related to disease severity were collected. Results: A total of 406 patients were included in the study. Ninety cases (22.17%) were Pneumonia. The most common clinical manifestations include Cough followed by fever. Factors associated with Pneumonia were male (adj odd ratio 2.79;95 CI 1.26-6.16; p-value 0.01 ), cardiovascular disease co-morbidity (odd ratio 2.97;95 CI 1.62-5.45; p-value 0.01) and sore throat ( adj odd ratio 0.22;95 CI 0.09-0.54;    p-value 0.001 ) and increase BMI each 1 kg/m 2 ( adj odd ratio 1.08;95 CI 1.01-1.14; p-value 0.009 ) diarrhea 4.04 ( Adj. Odds Ratio 4.04;95 CI 1.20-13.64, p-value= 0.024) hyposmia 8.37 ( Adj. Odds Ratio 8.37;95 CI 1.28-54.56, p-value= 0.026) parosmia 5.36     ( Adj. Odds Ratio 5.36;95 CI 1.10-26.05, p-value= 0.036) decrease systolic blood pressure each 1 mmHg. 0.97 ( Adj. Odds Ratio 0.97;95 CI 0.95-0.99; p-value= 0.032) Conclusions Patients with these risk factors should have early treatment and close monitoring.
