The development of Clinical Nursing Practice Guideline for Endovascular Aortic Aneurysm Repair Patient

This action research aimed to develop the clinical nursing practice guideline (CNPG) for abdominal aortic aneurysm (AAA) patients undergone endovascular aortic aneurysm repair (EVAR) and investigate the outcomes of the CNPG. The research was conducted in 2 phases: 1) Developing the CNPG according to the Soukup model and 2) Investigating the outcomes of the CNPG. The samples were 1) 35 registered nurses and 2) 25 AAA patients undergone EVAR.The research was conducted at Khon Kaen Hospital from 15 September 2023 to 31 May 2024. The research tools include 1) CNPG, 2) a nursing outcome assessment form, 3) a discipline in following the CNPG assessment form, 4) a nurse satisfaction assessment form and 5) a patient satisfaction assessment form. Data were analyzed by descriptive statistics. The results were as follows. 1) Four clinical nursing practices were obtained, namely preparation of the intensive care unit for receiving patients, postoperative critical care, discharge planning from the intensive care unit and nursing care for rehabilitation in postoperative period. 2) The innovation in receiving systematic patient information and EWS AAA KKH. 3) The results revealed that the nurses were satisfied with the overall CNPG at the highest level (M=90.40, SD=11.60). The patients were also satisfied with the nursing services at the highest level (M=4.60, SD=0.70). In addition, 98.32% of the nurses had discipline in following the CNPG. The risk incidence from not being prepared to receive patients, the complications caused by delay in recognizing early warning signs and reporting to doctors, pain score greater than 4 points in the first 24 hours after surgery of the group receiving the CNPG were less than those of the group not receiving the CNPG. Moreover, the extubation within the first 48 hours after surgery was more successful than the group not receiving the CNPG. In conclusion, The CNPG should be used continuously to achieve good results.
