The Development of a Clinical Supervision Model for Nursing Care of Older Patients with Surgery Hip Fracture at Uthai Thani Hospital

Abstract

This research and development aims to create a clinical supervision model and evaluate its effectiveness. The study was conducted in the orthopedic surgery ward and special ward of Uthai Thani Hospital from January 2, 2024, to July 31, 2024. The purposive sample consisted of 2 groups: 1) 22 professional nurses and 2) 36 elderly patients with hip fracture who underwent surgery. The research consisted of 3 phases: 1) a study of the analysis of the clinical supervision problem situation; 2) the development of a clinical supervision model; and 3) a study of the effectiveness of the supervision model. The data collection tools were a knowledge test, a supervisory attitude assessment form, a supervisor competency assessment form, a satisfaction assessment form with the model, and a nursing quality outcome record form. Data were analyzed using descriptive statistics and a dependent t-test. The results of the study found that the clinical supervision model developed based on Proctor's concept consisted of a supervisor potential development workshop, a manual for using the model, an action plan, and a follow-up evaluation of the clinical supervision and nursing quality. After using the clinical supervision model, it was found that the mean scores of knowledge and satisfaction of professional nurses were significantly higher than before using the model (p<0.05) The level of clinical supervisee competence was at a high level (X ̅ = 3.08, S.D.=0.29), but the attitude scores toward the supervision model were not significantly different at the 0.05 level. It can be seen that the developed clinical supervision model helped improve the outcomes in terms of nursing practice quality, reduce the number of hospital stays, costs, mortality rates, and reduce the risk of postoperative complications
