Development of a model for organizing the health service system for chronic non-communicable diseases in a primary care unit and health network. Bang Nam Priao District, Chachoengsao Province

This action research aimed to develop a model for organizing chronic noncommunicable disease services in the primary care unit network in Bang Nam Priao District, Chachoengsao Province, using an action research mechanism (PAOR). Participants included 35 interdisciplinary teams and community health networks and 390 NCD patients, selected purposively. Data were collected using in-depth interviews, observation and participation, questionnaires, and satisfaction assessments.The development was conducted from November 2024 to May 2025. The results were analyzed using content analysis, descriptive statistics, and a paired t-test. The results revealed that the model for organizing chronic noncommunicable disease services in the primary care unit network consisted of individualized patient care, patient-centered screening, and clearly defined roles and patient care systems with the participation of interdisciplinary professionals and community health networks. After using the model, the sample group showed a significant increase in knowledge of diabetes mellitus and hypertension (p < 0.05). Behavioral eating of sweet and salty foods. It was found that after the operation, the sample group had better eating habits for sweet and salty foods (p < 0.05), The development of the health service system for chronic non-communicable diseases in the primary care unit network can promote patients with chronic non-communicable diseases to be able to control their disease and have a better quality of life.
