Hip Fracture Situation and Associated Factors Among Older Adults in Nakhon Ratchasima Province

Background and Objective: Hip fracture is a major public health problem among older adults, significantly affecting functional independence, quality of life, and healthcare costs. This study aimed to describe the hip fracture situation and identify associated factors among older adults in Nakhon Ratchasima Province. Methods: A two-phase case-control study was conducted. Phase 1 reviewed medical records of 1,045 patients aged ≥ 60 years with hip fractures (ICD-10: S72.0–S72.2) from low energy trauma in public hospitals during 2023–2025. Phase 2 examined associated factors in 127 cases and 254 community-matched controls (1:2 ratio), using Chi-square test, Independent t-test, and Multiple Logistic Regression.
Results: The average incidence was 75.84 per 100,000 elderly population, declining from 87.49 (2023) to 57.94 (2025). Most patients were female (73.6%), mean age 79.0 years; trochanteric fractures were most prevalent (62.3%), and 52.7% had surgery within 2 days. Multiple Logistic Regression identified six independent factors: use of fall-risk medications (Adjusted OR = 5.42), physical inactivity (Adjusted OR = 3.87), inappropriate footwear (Adjusted OR = 3.82), improper use of walking aids (Adjusted OR = 3.34), female sex (Adjusted OR = 2.30), and having a caregiver (Adjusted OR = 2.52); all p < 0.05. Discussion and Recommendations: The three leading modifiable risk factors 1.Use of fall-risk medications 2. Physical inactivity and 3. Inappropriate footwear should be priority targets for proactive prevention through routine medication review, exercise promotion, and footwear counseling in elderly care settings.
