Role of Nurse on Team-Based Care for Improving Blood Pressure Control in Older Patients

Abstract

         Nurses have been recognized as a profession to improving hypertension control. This article aims to present the role of the nurses in team-based hypertension care, including: 1) detection, referral, and follow up; 2) diagnostics and medication management; 3) patient education, counseling, and skill building; 4) coordination of care; 5) clinic management; and 6) performance measurement and quality improvement. Nurses’ roles in team-based hypertension care can improve patient outcomes and quality of care.
