การรับรู้ของพยาบาลต่อการใช้เครื่องมือสังเกตความปวดสำหรับการดูแลผู้ป่วยสมองบาดเจ็บภาวะวิกฤตฉบับภาษาไทยในผู้ป่วยศัลยกรรมประสาท: การวิจัยเชิงคุณภาพ

บทคัดย่อ

วัตถุประสงค์: เพื่อศึกษาการรับรู้ของพยาบาลต่อการใช้เครื่องมือสังเกตความปวดสำหรับการดูแลผู้ป่วยสมองบาดเจ็บภาวะวิกฤตฉบับภาษาไทย


รูปแบบการวิจัย: การวิจัยเชิงคุณภาพแบบบรรยาย


วิธีดำเนินการวิจัย: การศึกษาครั้งนี้ใช้วิธีการสัมภาษณ์แบบกึ่งโครงสร้างโดยใช้คำถามปลายเปิด พยาบาลวิชาชีพจำนวน 10 ราย ที่ผ่านการฝึกอบรมและมีประสบการณ์ในการใช้เครื่องมือสังเกตความปวดสำหรับการดูแลผู้ป่วยสมองบาดเจ็บภาวะวิกฤตฉบับภาษาไทย ได้รับการเชิญชวนให้เข้าร่วมวิจัยและให้ข้อมูลจนมีความอิ่มตัว การสัมภาษณ์ใช้เวลา 20-40 นาที วิเคราะห์ข้อมูลโดยใช้การวิเคราะห์เนื้อหาเพื่อสรุปเป็นความหมายของผลการศึกษา


ผลการวิจัย: ผลการวิเคราะห์ข้อมูลพบว่าการรับรู้ของพยาบาลต่อการใช้เครื่องมือสังเกตความปวดสำหรับการดูแลผู้ป่วยสมองบาดเจ็บภาวะวิกฤตฉบับภาษาไทยจัดกลุ่มได้เป็น 3 หมวดหมู่หลักและ 6 หมวดหมู่ย่อย ได้แก่: 1) การยอมรับ มี 2 หมวดหมู่ย่อย — ใช้ง่าย/พึงพอใจสูง และไม่เหมาะที่จะใช้กับเงื่อนไขเฉพาะ; 2) ความเป็นไปได้ มี 2 หมวดหมู่ย่อยการฝึกอบรมที่มีประสิทธิภาพ และ มีเงื่อนไขจำกัด; และ 3) อรรถประโยชน์ทางคลินิก มี 2 หมวดหมู่ย่อยคุ้มค่าที่จะนำไปปฏิบัติ และขาดการมีส่วนร่วม ผู้ให้ข้อมูลส่วนหนึ่งเสนอให้มีการปรับปรุงการให้คะแนนเพื่อให้การคำนวนคะแนนรวมทั้งหมดแม่นยำมากขึ้นในกรณีที่มีรายการขาดหายไป การฝึกอบรมแบบสหสาขาวิชาชีพมีความสำคัญที่จะเพิ่มความถูกต้องแม่นยำในการใช้เครื่องมือ และเห็นด้วยที่จะนำเครื่องมือนี้มาประยุกต์ใช้ในการประเมินความปวดในผู้ป่วยศัลยกรรมประสาทแทนการใช้เครื่องมือเดิม


สรุปและข้อเสนอแนะ: เครื่องมือสังเกตความปวดสำหรับการดูแลผู้ป่วยสมองบาดเจ็บภาวะวิกฤตฉบับภาษาไทย มีความเหมาะสม มีประโยชน์ และใช้ได้ง่ายสำหรับประเมินความปวดในผู้ป่วยศัลยกรรมประสาททั้งในและนอกหออภิบาลผู้ป่วย การประยุกต์ใช้ในบริบทของไทยร่วมกับการฝึกอบรมแบบสหสาขาวิชาชีพจะช่วยเพิ่มการประเมินความปวดที่ดีที่สุดให้แก่ผู้ป่วย
