บทคัดย่อ

ภาวะเหนื่อยล้าที่เกิดขึ้นในผู้ป่วยภาวะโพสท์โควิด-19 อาจนำไปสู่อาการที่เรื้อรังที่ส่งผลกระทบต่อการดำเนินชีวิตตามมา และยังมีผลต่อความสามารถในการทำหน้าที่ลดลง ทำให้คุณภาพชีวิตแย่ลงเป็นระยะเวลานาน


วัตถุประสงค์: เพื่อศึกษาอำนาจการทำนายของอายุ ดัชนีมวลกาย ความรุนแรงของโรคโควิด-19 และภาวะซึมเศร้าต่อภาวะเหนื่อยล้าในผู้ป่วยภาวะโพสท์โควิด-19


รูปแบบการวิจัย: การศึกษารูปแบบความสัมพันธ์เชิงทำนาย


วิธีดำเนินการวิจัย: กลุ่มตัวอย่างคัดเลือกแบบสะดวกเป็นผู้ป่วยที่มีภาวะโพสท์โควิด-19 จำนวน 120 คน ทั้งเพศชายและเพศหญิง ที่มาติดตามการรักษาในหน่วยตรวจโรคอายุรศาสตร์แผนกผู้ป่วยนอกโรงพยาบาลตติยภูมิขั้นสูงแห่งหนึ่งในกรุงเทพมหานคร โดยต้องมีคุณสมบัติ คือ อายุ 18 ปีขึ้นไป และสามารถพูด อ่านและเขียนภาษาไทยได้ ระยะเวลาเก็บข้อมูลตั้งแต่เดือนกันยายนถึงเดือนพฤศจิกายน พ.ศ. 2566 เก็บรวบรวมข้อมูลโดยใช้แบบบันทึกข้อมูลส่วนบุคคลและประวัติการเจ็บป่วยของผู้ป่วยโพสต์โควิด-19 แบบประเมินภาวะเหนื่อยล้า แบบประเมินภาวะซึมเศร้า วิตกกังวลและความเครียด วิเคราะห์ข้อมูลโดยใช้สถิติพรรณนา และการวิเคราะห์ถดถอยเชิงพหุคูณ


ผลการวิจัย: กลุ่มตัวอย่างมีอายุเฉลี่ย 53.88 ปี (SD = 13.75) เพศหญิงร้อยละ 78.3 กลุ่มตัวอย่างมีภาวะเหนื่อยล้ามากร้อยละ 21.7 และมีภาวะเหนื่อยล้าน้อยร้อยละ 78.3 อายุ ดัชนีมวลกาย ความรุนแรงของโรคโควิด-19 และภาวะซึมเศร้าสามารถร่วมกันทำนายภาวะเหนื่อยล้าในผู้ป่วยภาวะโพสท์โควิด-19 ได้ร้อยละ 33.2 (R2 = .33) โดยพบว่า อายุ และภาวะซึมเศร้าเท่านั้นที่สามารถทำนายภาวะเหนื่อยล้าในผู้ป่วยภาวะโพสท์โควิด-19 ได้อย่างมีนัยสำคัญ (equation = - .22, p < .05 และ equation = - .54, p < .001 ตามลำดับ)


สรุปและข้อเสนอแนะ: อายุและภาวะซึมเศร้าอาจส่งผลต่อภาวะเหนื่อยล้า เพื่อป้องกันหรือควบคุมอาการดังกล่าว ทีมดูแลสุขภาพควรมีการประเมินภาวะเหนื่อยล้า และภาวะซึมเศร้าในผู้ป่วยภาวะโพสท์โควิด-19 โดยเฉพาะประเมินผู้ป่วยสูงวัยทุกราย เพื่อให้ทราบปัญหาและแก้ไขได้ทันในระหว่างที่ผู้ป่วยได้รับการรักษา และควรมีพยาบาลหรือบุคลากรในทีมสุขภาพให้ความรู้ คำแนะนำ หาแนวทางช่วยเหลือ ป้องกันภาวะซึมเศร้าในผู้ป่วยภาวะโพสท์โควิด-19 เพื่อลดภาวะเหนื่อยล้าจนกระทั่งผู้ป่วยฟื้นหาย
