ปัจจัยทำนายภาวะไตบาดเจ็บเฉียบพลันของผู้ป่วยติดเชื้อในกระแสเลือดที่เข้ารับการรักษาในแผนกอุบัติเหตุ-ฉุกเฉิน

บทคัดย่อ
วัตถุประสงค์: เพื่อศึกษาอำนาจการทำนายของความรอบรู้ด้านสุขภาพ การรับรู้ความรุนแรงของโรค การสนับสนุนจากครอบครัว และระยะเวลาตั้งแต่รับรู้อาการจนมาถึงโรงพยาบาล ต่อภาวะไตบาดเจ็บเฉียบพลันในผู้ป่วยติดเชื้อในกระแสเลือดที่เข้ารับการรักษาในแผนกอุบัติเหตุ-ฉุกเฉิน
รูปแบบการวิจัย: การศึกษาความสัมพันธ์เชิงทำนายแบบภาคตัดขวาง
วิธีดำเนินการวิจัย: กลุ่มตัวอย่างเป็นผู้ป่วยติดเชื้อในกระแสเลือด อายุ 18 ปีขึ้นไป ทั้งเพศชายและหญิง ที่เข้ารับการรักษาในแผนกอุบัติเหตุ-ฉุกเฉิน โรงพยาบาลระดับตติยภูมิแห่งหนึ่งในกรุงเทพมหานคร คัดเลือกกลุ่มตัวอย่างแบบสะดวก จำนวน 151 ราย เก็บรวบรวมข้อมูลโดยใช้แบบประเมินสมรรถภาพสมอง แบบบันทึกข้อมูลทั่วไป แบบประเมินภาวะไตบาดเจ็บเฉียบพลัน แบบประเมินความรอบรู้ด้านสุขภาพการเจ็บป่วยฉุกเฉิน แบบประเมินการรับรู้ความรุนแรงของโรค และแบบประเมินการสนับสนุนจากครอบครัว วิเคราะห์ข้อมูลโดยใช้สถิติพรรณนาและสถิติถดถอยโลจิสติก
ผลการวิจัย: ความรอบรู้ด้านสุขภาพ การรับรู้ความรุนแรงของโรค การสนับสนุนจากครอบครัว และระยะเวลาตั้งแต่รับรู้อาการจนมาถึงโรงพยาบาลสามารถร่วมทำนายภาวะไตบาดเจ็บเฉียบพลันในผู้ป่วยติดเชื้อในกระแสเลือดได้โดยอธิบายความแปรปรวนได้ร้อยละ 45 (Nagelkerke R2 = .45) โดยกลุ่มตัวอย่างที่มีความรอบรู้ด้านสุขภาพในระดับปานกลางและเพียงพอมีโอกาสเกิดภาวะไตบาดเจ็บเฉียบพลันลดลง ร้อยละ 84 (OR = .16, 95%CI [.05, .52], p = .002) และร้อยละ 95 (OR = .05, 95%CI [.01, .17], p < .001) ตามลำดับ กลุ่มตัวอย่างที่มีการรับรู้ความรุนแรงของโรคระดับสูงมีโอกาสเกิดภาวะไตบาดเจ็บเฉียบพลันลดลง ร้อยละ 62 (OR = .38, 95%CI [.15, .99], p = .05) ขณะที่กลุ่มตัวอย่างที่มีระยะเวลาตั้งแต่รับรู้อาการจนมาถึงโรงพยาบาลมากกว่า 72 ชั่วโมงและมากกว่า 120 ชั่วโมง มีโอกาสเกิดภาวะไตบาดเจ็บเฉียบพลันเพิ่มขึ้นประมาณ 4 เท่า (OR = 4.09, 95%CI [1.12, 14.88], p = .033 และ OR = 3.92, 95%CI [1.27, 12.15], p = .02 ตามลำดับ) ส่วนการสนับสนุนจากครอบครัวถึงแม้มีความสัมพันธ์กับภาวะไตบาดเจ็บเฉียบพลัน (c2 = 10.07, p = .007) แต่ไม่สามารถทำนายภาวะไตบาดเจ็บเฉียบพลันได้เมื่อควบคุมตัวแปรอื่นที่ศึกษา
สรุปและข้อเสนอแนะ: พยาบาลและบุคลากรทางการแพทย์ควรนำผลการวิจัยครั้งนี้ไปใช้ในการวางแผนการดูแลผู้ป่วยติดเชื้อในกระแสเลือด โดยมุ่งเน้นการส่งเสริมความรอบรู้ด้านสุขภาพ การรับรู้ความรุนแรงของโรค และการมีส่วนร่วมของครอบครัว ตั้งแต่ระยะแรกของการติดเชื้อในกระแสเลือดเพื่อส่งเสริมการเข้ารับการรักษาอย่างทันท่วงที ลดความรุนแรงของโรค และป้องกันการเกิดภาวะไตบาดเจ็บเฉียบพลันในอนาคต
