บทคัดย่อ

วัตถุประสงค์: เพื่อศึกษาอิทธิพลของปัจจัยรายได้ ความรอบรู้ด้านสุขภาพ การเข้าถึงบริการสุขภาพ บริเวณที่อยู่อาศัย และการสนับสนุนของครอบครัว ต่อคุณภาพชีวิตของผู้ป่วยเบาหวานชนิดที่ 2 ที่มีภาวะน้ำหนักเกินและมีโรคอ้วน


รูปแบบการวิจัย: รูปแบบการวิจัยเชิงทำนายแบบตัดขวาง


วิธีดำเนินการวิจัย: กลุ่มตัวอย่างเป็นผู้ป่วยที่ได้รับการวินิจฉัยจากแพทย์ว่าเป็นโรคเบาหวานชนิดที่ 2 ที่มีภาวะน้ำหนักเกิน และ/หรือ มีโรคอ้วน จำนวน 101 ราย ที่มารับบริการคลินิกเบาหวานในโรงพยาบาลตติยภูมิแห่งหนึ่งในภาคตะวันออก เกณฑ์คัดเข้า ได้แก่ อายุ 20 ปีขึ้นไป มีค่า BMI มากกว่าหรือเท่ากับ 23 kg/m2 ขึ้นไป เป็นเบาหวานชนิดที่ 2 ไม่น้อยกว่า 3 เดือน และได้รับยาลดระดับน้ำตาลในเลือดชนิดรับประทาน เครื่องมือที่ใช้ในการวิจัย ประกอบด้วย แบบสอบถามข้อมูลส่วนบุคคลและแบบบันทึกข้อมูลสุขภาพ แบบสอบถามความรอบรู้ด้านสุขภาพ แบบสอบถามด้านการเข้าถึงบริการสุขภาพ แบบสอบถามบริเวณที่อยู่อาศัย แบบสอบถามการสนับสนุนของครอบครัว และแบบสอบถามคุณภาพชีวิตของผู้ป่วยเบาหวาน วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนาและการวิเคราะห์ถดถอยพหุคูณแบบขั้นตอน


ผลการวิจัย: ผู้เข้าร่วมวิจัยทั้งหมดมีอายุเฉลี่ย 54.24 ปี (SD = 9.04) และร้อยละ 54.5 เป็นเพศหญิง มีคุณภาพชีวิตที่ดี ร้อยละ 86.1 ความรอบรู้ด้านสุขภาพที่เพียงพอ ร้อยละ 77.2 การเข้าถึงบริการสุขภาพอยู่ในระดับสูง ร้อยละ 91.1 มีทรัพยากรสภาพแวดล้อมบริเวณที่อยู่อาศัยในระดับมาก ร้อยละ 89.1 มีการสนับสนุนของครอบครัวอยู่ในระดับดี ร้อยละ 100 โดยความรอบรู้ด้านสุขภาพ (β = .21, p < .05) การเข้าถึงบริการสุขภาพ (β = .20, p < .05) และการสนับสนุนของครอบครัว (β = .20, p < .05) สามารถทำนายคุณภาพชีวิตได้อย่างมีนัยสำคัญ และสามารถร่วมกันอธิบายความผันแปรของคุณภาพชีวิต ได้ร้อยละ 15 (R2 = .15)


สรุปและข้อเสนอแนะ: จากผลการวิจัยครั้งนี้สนับสนุนกรอบแนวคิดปัจจัยสังคมกำหนดสุขภาพ พบว่าปัจจัยความรอบรู้ด้านสุขภาพ การเข้าถึงบริการสุขภาพ และการสนับสนุนของครอบครัว สามารถทำนายคุณภาพชีวิตของผู้ป่วยเบาหวานชนิดที่ 2 ที่มีน้ำหนักเกินและมีโรคอ้วนได้ พยาบาลควรจัดกิจกรรมส่งเสริมความรอบรู้ด้านสุขภาพ เอื้ออำนวยการเข้าถึงบริการสุขภาพในกลุ่มผู้ป่วยนี้ รวมถึงการส่งเสริมให้ครอบครัวเข้ามามีส่วนร่วมมากขึ้นด้วย
