การตัดสินใจเชิงจริยธรรมของพยาบาลผู้จัดการรายกรณี โรคเรื้อรังโรงพยาบาลทั่วไปในภาคใต้ของประเทศไทย

บทคัดย่อ
วัตถุประสงค์: การวิจัยเชิงคุณภาพครั้งนี้ มีวัตถุประสงค์เพื่อศึกษาการตัดสินใจเชิงจริยธรรมของพยาบาลผู้จัดการรายกรณีโรคเรื้อรัง โรงพยาบาลทั่วไปในภาคใต้ของประเทศไทย วิธีการ: โดยการวิเคราะห์เหตุการณ์สำคัญ ผู้ให้ข้อมูลเป็นพยาบาลผู้จัดการรายกรณีโรคเรื้อรัง จำนวน 32 ราย เก็บข้อมูลโดยการสัมภาษณ์รายบุคคล ด้วยคำถามปลายเปิดวิเคราะห์ข้อมูลโดยการวิเคราะห์เหตุการณ์สำคัญของฟลานาแกน ผลการศึกษา: 1) เหตุการณ์สำคัญที่พยาบาลผู้จัดการรายกรณีโรคเรื้อรังต้องเผชิญในขณะปฏิบัติหน้าที่ และเกิดประเด็นขัดแย้งทางจริยธรรม คือ แผนการรักษาไม่สอดคล้องกับความต้องการของผู้ป่วยและญาติ แม้ว่าการรักษานั้นเป็นไปตามมาตรฐาน (57.57%) ผู้รับบริการไม่มีเงินจ่ายค่ารักษาพยาบาล/ซื้ออุปกรณ์ (30.30%) และผู้รับบริการไม่ได้รับการดูแลตามแนวปฏิบัติการรักษา (12.12%) 2) รูปแบบการตัดสินใจเชิงจริยธรรมของพยาบาลผู้จัดการรายกรณีโรคเรื้อรังที่มีจำนวนมากที่สุด คือ ปรึกษากับญาติผู้ดูแล/ทีมสหวิชาชีพ (40.90%) รองลงมา คือ หาแหล่งสนับสนุน/ประสานผู้ที่เกี่ยวข้องเพื่อช่วยเหลือผู้ป่วยให้ได้รับการรักษาตามแผนการรักษา/ตามมาตรฐาน (27.27%) และตัดสินใจทำหน้าที่แทนผู้ป่วยในการเจรจาต่อรองกับแพทย์ (19.70%) 3) เหตุผล/หลักการที่พยาบาลผู้จัดการรายกรณีโรคเรื้อรังใช้ในการตัดสินใจเชิงจริยธรรมที่มีจำนวนมากที่สุด คือ ประโยชน์/ความปลอดภัยของผู้ป่วย (33.30%) รองลงมา คือ การเคารพสิทธิผู้ป่วย/ญาติ (30.30%) และมาตรฐาน/คุณภาพการพยาบาล/การรักษาที่ดี (21.20%) อภิปรายผล/สรุป: ผลการวิจัยสามารถนำมาเป็นข้อมูลพื้นฐานสำหรับผู้บริหารทางการพยาบาลใช้เป็นแนวทางในการพัฒนาความรู้และทักษะแก่พยาบาลผู้จัดการรายกรณีโรคเรื้อรังให้สามารถตัดสินใจเชิงจริยธรรมได้อย่างมีประสิทธิภาพ
