การศึกษาเปรียบเทียบผลลัพธ์ของการตั้งครรภ์และการคลอด ระหว่างสตรีตั้งครรภ์วัยรุ่นและสตรีตั้งครรภ์วัยผู้ใหญ่: กรณีศึกษาของโรงพยาบาลศูนย์แห่งหนึ่งในภาคใต้

บทคัดย่อ
วัตถุประสงค์: เปรียบเทียบผลลัพธ์ทางสูติศาสตร์ (การตั้งครรภ์และการคลอด) ในสตรีตั้งครรภ์วัยรุ่นกับผู้ใหญ่ วิธีการ: เก็บข้อมูลย้อนหลังเชิงวิเคราะห์แบ่งเป็นกลุ่มศึกษาคือ วัยรุ่น (อายุน้อยกว่า 20 ปี) และกลุ่มเปรียบเทียบวัยผู้ใหญ่ที่มาคลอดบุตรในโรงพยาบาลหาดใหญ่ปี พ.ศ. 2559-2563 รวบรวมภาวะแทรกซ้อนที่เกิดขึ้นระหว่างตั้งครรภ์จนถึงหลังคลอดบุตร วิเคราะห์ข้อมูล ได้แก่ ความถี่ร้อยละ ทดสอบความสัมพันธ์โดยใช้การทดสอบไคสแควร์ และหาค่าความเสี่ยงสัมพัทธ์ที่ระดับ ความความเชื่อมั่น ร้อยละ 95 และความแตกต่างของสัดส่วนโดยมีนัยสำคัญทางสถิติที่ p < .05 ผลการศึกษา: พบสตรีตั้งครรภ์วัยรุ่น (อายุ 13-19 ปี) ร้อยละ 9.40 ของการคลอดทั้งหมด (2,885 คน) โดยภาวะแทรกซ้อนทางสูติศาสตร์ พบในวัยรุ่นสูงกว่าวัยผู้ใหญ่ที่มีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติได้แก่ ภาวะคลอดก่อนกำหนด (RR 1.47; 95% CI 1.31-1.47) ภาวะถุงน้ำคร่ำแตกก่อนกำหนด (RR 1.29; 95% CI 1.02-1.40) และการติดเชื้อของระบบสืบพันธุ์และทางเดินปัสสาวะระหว่างตั้งครรภ์ (RR 2.51; 95% CI 1.90-3.72) ส่วนผลการคลอดของทารกพบในวัยรุ่นสูงกว่าวัยผู้ใหญ่ที่มีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติ ได้แก่ภาวะทารกน้ำหนักตัวน้อย (RR 1.58; 95% CI 1.46-2.02) การบาดเจ็บที่หนังศีรษะจากการคลอด (RR 1.45; 95% CI 1.18-1.94) ภาวะปอดอักเสบแต่กำเนิด (RR 1.50; 95% CI 1.06-1.80) ทารกตัวเล็กกว่าอายุครรภ์ (RR 1.63; 95% CI 1.25-2.31) และภาวะโรคแทรกของรก สายสะดือ และเยื่อถุงน้ำคร่ำ (RR 2.44; 95% CI 1.18-3.93)สรุป: การศึกษาครั้งนี้ชี้ให้เห็นว่า ภาวะแทรกซ้อนของการตั้งครรภ์ในวัยรุ่นสูงกว่าวัยผู้ใหญ่ ดังนั้นบุคลากรสุขภาพควรตระหนักถึงวิธีแก้ไขปัญหาและป้องกันภาวะแทรกซ้อนที่พบในวัยรุ่น
