ปัจจัยเสี่ยงต่อการติดเชื้อเยื่อบุช่องท้องในผู้ป่วยล้างไตทางช่องท้อง: โรงพยาบาลวชิระภูเก็ต

บทคัดย่อ
วัตถุประสงค์: เพื่อศึกษาอุบัติการณ์สะสม ลักษณะทางคลินิก ปัจจัยเสี่ยงต่อการติดเชื้อในเยื่อบุช่องท้อง และผลลัพธ์การรักษาการติดเชื้อในเยื่อบุช่องท้องของผู้ป่วยล้างไตทางช่องท้อง วิธีการ: ศึกษาย้อนหลังแบบบรรยายในผู้ป่วยล้างไตทางช่องท้องของโรงพยาบาลวชิระภูเก็ตปีงบประมาณ 2564-2566 เครื่องมือเก็บรวบรวมข้อมูลผ่านการตรวจสอบความตรงและความเที่ยง วิเคราะห์ข้อมูลด้วยสถิติไคสแควร์และสหสัมพันธ์แบบพอยท์ไบซีเรียล เพื่อคัดเลือกตัวแปรเข้าสู่สมการทำนายโดยใช้สถิติการถดถอยโลจิสติกแบบไบนารี ผลการศึกษา: ผู้ป่วยล้างไตทางช่องท้อง ปีงบประมาณ 2564-2566 มีจำนวนสะสม 481 คน เกิดการติดเชื้อในเยื่อบุช่องท้อง 191 คน (ร้อยละ 39.70) โดยมีจำนวนครั้งของการติดเชื้อ 1-5 ครั้ง เฉลี่ย 1.54 ครั้ง/คน พบว่า ปัจจัยที่สัมพันธ์กับการติดเชื้อในเยื่อบุช่องท้องได้แก่ ระดับอัลบูมินในเลือด จำนวนปัสสาวะต่อวัน การทำความสะอาดบ้านทุกวัน การมีสัตว์เลี้ยงในบ้าน การล้างมือ การสวมหน้ากากอนามัย การปฏิบัติตามขั้นตอน การล้างไต และสถานที่เปลี่ยนน้ำยาล้างไต ผลการวิเคราะห์ปัจจัยทำนายพบว่า การล้างมือและการสวมหน้ากากอนามัยเป็นปัจจัยสำคัญในการป้องกันการติดเชื้อเยื่อบุช่องท้อง (ปี 2564 Odds ratio = .073, 95%CI =.019, .284; ปี 2565 Odds ratio =.061, 95%CI =.020, .183; ปี 2566 Odds ratio =.046, 95%CI =.013, .155) โดยตัวแปรที่นำเข้าสู่สมการทำนายร่วมกันอธิบายการเกิดการติดเชื้อเยื่อบุช่องท้อง ร้อยละ 49.6 ถึง 52.9 ค่ามัธยฐานค่าใช้จ่ายในการรักษาการติดเชื้อในเยื่อบุช่องท้อง 17,619.27 บาท ถึง 43,660 บาท สรุป: การติดเชื้อในเยื่อบุช่องท้องป้องกันได้โดยการปรับพฤติกรรมการล้างมือและการสวมหน้ากากอนามัย มีการติดตามการปฏิบัติตามขั้นตอนการล้างไตอย่างใกล้ชิดและเยี่ยมบ้านโดยการมีส่วนร่วมของเครือข่ายในชุมชนเพื่อให้มั่นใจถึงการปฏิบัติตามแนวทางที่ถูกต้อง ซึ่งจะช่วยให้ผู้ป่วยปลอดภัยและมีคุณภาพชีวิตที่ดี
