บทคัดย่อ
โรคหลอดเลือดสมองเป็นภาวะฉุกเฉินทางการแพทย์ที่ต้องได้รับการประเมินอย่างถูกต้อง รวดเร็วเพื่อให้ผู้ป่วยได้รับการรักษาทันท่วงที พยาบาลห้องฉุกเฉินโรงพยาบาลระดับต้นมีบทบาทสำคัญต่อการประเมินอาการเบื้องต้นเพื่อจัดการภาวะคุกคามชีวิตของผู้ป่วย ภายใต้ข้อจำกัดด้านทรัพยากร บุคลากรและเครื่องมือแพทย์ บทความนี้มีวัตถุประสงค์เพื่อพัฒนาแนวทางการจัดการทางการพยาบาลในผู้ป่วยโรคหลอดเลือดสมองเฉียบพลัน และศึกษาผลลัพธ์ทางการพยาบาลจากการทดลอง
ใช้แผนการพยาบาลตามกรอบแนวคิดแนวปฏิบัติการพยาบาลจากหลักฐานเชิงประจักษ์ของซูคัพ ประกอบด้วย การค้นหาปัญหาทางคลินิก การสืบค้นหลักฐานเชิงประจักษ์ตามกรอบ PICO-T ประเมินระดับความน่าเชื่อถือของหลักฐานเชิงประจักษ์โดยใช้เกณฑ์ของสถาบันโจแอนนาบริกส์ พัฒนาแนวทางการพยาบาลจากหลักฐานเชิงประจักษ์จำนวน 17 เรื่อง ครอบคลุมแนวทางการจัดการทางการพยาบาล ประกอบด้วย 4 องค์ประกอบหลัก ได้แก่ 1. การคัดกรองผู้ป่วย 2. การประเมินอาการและ
ภาวะคล้ายโรคหลอดเลือดสมอง 3. การจัดการทางการพยาบาลในห้องฉุกเฉิน และ 4. การประสานการส่งต่อ จากนั้นนำไปทดลองใช้กับผู้ป่วยสงสัยโรคหลอดเลือดสมองที่ห้องฉุกเฉิน โรงพยาบาลระดับต้นแห่งหนึ่ง จำนวน 5 ราย ติดตามผลลัพธ์หลังจากการจัดการทางการพยาบาล พบว่า พยาบาลคัดกรองได้ถูกต้อง ระยะเวลาการรายงานแพทย์ลดลง ไม่พบอาการผู้ป่วยทรุดลงขณะรอส่งต่อ ระยะเวลาตั้งแต่แรกรับถึงส่งต่อลดลงเมื่อเทียบกับการจัดการทางการพยาบาลแบบเดิม ผลลัพธ์แสดงให้เห็นว่า แนวทางนี้สามารถดูแลผู้ป่วยได้รวดเร็วขึ้นและนำไปใช้ได้จริงในโรงพยาบาลระดับต้น อย่างไรก็ตาม ควรให้การพยาบาลตาม
ลักษณะเฉพาะของผู้ป่วยเพื่อลดอัตราการเสียชีวิตและภาวะทุพพลภาพ
