การพัฒนารูปแบบการจัดการรายกรณีของผู้ป่วยจิตเวชยุ่งยากซับซ้อน: กรณีศึกษาอำเภอแว้ง

บทคัดย่อ
การวิจัยและพัฒนามีวัตถุประสงค์เพื่อพัฒนาและทดลองใช้รูปแบบการดูแลแบบการจัดการรายกรณีสำหรับผู้ป่วยจิตเวชยุ่งยากซับซ้อนในชุมชน แบ่งออกเป็น 2 ระยะ คือ 1) ระยะพัฒนารูปแบบการจัดการรายกรณีผู้ป่วยจิตเวชยุ่งยากซับซ้อนในชุมชน โดยใช้กรอบแนวคิดของโพเวลร่วมกับการทบทวนวรรณกรรมที่เกี่ยวข้องกับการจัดการรายกรณี และ
การวิเคราะห์ข้อมูลเชิงคุณภาพ การออกแบบแนวทางการดูแลแบบการจัดการรายกรณีและการตรวจสอบคุณภาพ ด้วยผู้ทรงคุณวุฒิ 3 ท่าน ค่า IOC = .81 และ 2) ระยะทดลองใช้รูปแบบการจัดการรายกรณีฯ โดยทดลองในกลุ่มตัวอย่างพยาบาลวิชาชีพ จำนวน 6 คน วิเคราะห์ข้อมูลเชิงคุณภาพด้วยการวิเคราะห์เชิงเนื้อหาและข้อมูลเชิงปริมาณด้วยสถิติพรรณนา ผลการวิจัย: รูปแบบการจัดการรายกรณีของผู้ป่วยจิตเวชยุ่งยากซับซ้อนสำหรับพยาบาลในชุมชน พัฒนาจากแนวคิดของโพเวล การทบทวนวรรณกรรม และผลการวิเคราะห์ข้อมูลเชิงคุณภาพ ประกอบด้วย 1) องค์ประกอบของการจัดการรายกรณี คือ ผู้จัดการรายกรณีที่เป็นพยาบาลวิชาชีพ ทีมการจัดการรายกรณีในชุมชน และแผนการดูแลผู้ป่วยจิตเวชยุ่งยากซับซ้อน 2) คู่มือแนวปฏิบัติการจัดการรายกรณีผู้ป่วยจิตเวชยุ่งยากซับซ้อน และ 3) กระบวนการดำเนินการจัดการรายกรณี ผ่านเครื่องมือระบบ application IMed@home และได้ทดลองใช้รูปแบบการจัดการรายกรณี เป็นระยะเวลา 6 เดือน พบว่า ผู้ป่วยไม่มีอาการทางจิต ไม่ต้องเข้ารับการรักษาในโรงพยาบาลซ้ำ และภาพรวมของระดับความพึงพอใจของการนำไปใช้โดยพยาบาลอยู่ในระดับดีมาก สรุป: รูปแบบการจัดการรายกรณีสำหรับผู้ป่วยจิตเวชยุ่งยากซับซ้อน
ในชุมชน สามารถนำไปใช้สำหรับพัฒนาคุณภาพการดูแลผู้ป่วยจิตเวชยุ่งยากซับซ้อนในชุมชนได้
