ผลของโปรแกรมการเรียนรู้แบบผสมผสานต่อประสิทธิภาพการวินิจฉัยการติดเชื้อในโรงพยาบาล ของพยาบาลควบคุมการติดเชื้อประจำหอผู้ป่วย

บทคัดย่อ

วัตถุประสงค์: เพื่อเปรียบเทียบประสิทธิภาพในการวินิจฉัยการติดเชื้อในโรงพยาบาลของพยาบาลควบคุมการติดเชื้อประจำหอผู้ป่วย (infection control ward nurse [ICWN]) กลุ่มทดลอง ก่อนและหลังได้รับโปรแกรมการเรียนรู้แบบผสมผสานและระหว่างกลุ่ม วิธีการ: การวิจัยกึ่งทดลองแบบสองกลุ่มวัดก่อนและหลัง กลุ่มตัวอย่างคือ ICWN แบ่งเป็นกลุ่มทดลองและ กลุ่มควบคุมกลุ่มละ 25 คน เครื่องมือวิจัยประกอบด้วย 1) โปรแกรมการเรียนรู้แบบผสมผสานในการวินิจฉัยการติดเชื้อในโรงพยาบาล (4 สัปดาห์) และ 2) แบบบันทึกการวินิจฉัยการติดเชื้อในโรงพยาบาลจากสถานการณ์จำลอง ตรวจสอบความตรงตามเนื้อหาจากผู้ทรงคุณวุฒิจำนวน 6 ท่าน แบบบันทึกการวินิจฉัยผ่านการทดสอบโดยหาความสอดคล้องระหว่างผู้วิจัยและพยาบาลควบคุมการติดเชื้อ ได้ค่า Cohen's kappa = 1.00 วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา เปรียบเทียบความแตกต่างการวินิจฉัยการติดเชื้อในโรงพยาบาลภายในกลุ่มด้วย paired samples t-test, Wilcoxon signed rank test และระหว่างกลุ่มด้วย Independent samples t-test, Mann–Whitney U test ผลการศึกษา: หลังได้รับโปรแกรม กลุ่มทดลองมีค่าความถูกต้องในการวินิจฉัยการติดเชื้อในโรงพยาบาลเพิ่มขึ้นอย่างมีนัยสำคัญทางสถิติ จากร้อยละ 58.00 เป็น 82.25 (p < .001) ค่าความไวเพิ่มขึ้นจากร้อยละ 53.71 เป็น 80.57 (p < .001) ขณะที่ค่าความจำเพาะเพิ่มขึ้นจาก ร้อยละ 88.00 เป็น 94.00 (p = .180) และค่าพยากรณ์บวกเพิ่มขึ้นจากร้อยละ 96.91 เป็น 98.89 (p = .093) แต่ไม่มีนัยสำคัญทางสถิติ และสูงกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติ (p < .001) สรุป: โปรแกรมการเรียนรู้แบบผสมผสาน สามารถนำไปประยุกต์ใช้ในการเสริมสร้างความรู้และเพิ่มประสิทธิภาพในการวินิจฉัยการติดเชื้อในโรงพยาบาลของ ICWN ในบริบทที่ใกล้เคียงกันได้
