การประเมิน-คัดกรองภาวะทุพโภชนาการในผู้ป่วย (ผู้ใหญ่) ตามแนวความคิดเห็นใหม่ของสากล จากแบบฟอร์ม BNT 2000 เป็น NT 2013

บทคัดย่อ
การประเมิน-คัดกรองภาวะทุพโภชนาการ เป็นขั้นตอนเบื้องต้นของการดูแลรักษาผู้ป่วยด้านโภชนบำบัดอย่างมีคุณภาพ ในปี พ.ศ. 2543 (ค.ศ. 2000) ผู้เขียนได้นำเสนอแบบฟอร์มการประเมิน-คัดกรองฯ BNT (Bhumibol Adulyadej Hospital Nutrition Triage)(1-5) และได้ศึกษาปรับปรุงรูปแบบมาตามลำดับ ในปี พ.ศ. 2545 (ค.ศ. 2002) จึงศึกษาวิจัยเปรียบเทียบ BNT กับแบบประเมิน SGA และในปี พ.ศ. 2555 (ค.ศ. 2012) ได้ทำการเรียบเรียงเนื้อหาและปรับเกณฑ์การประเมินในแบบฟอร์มการประเมินฯ ให้สอดคล้องกันกับมติสากล (Consensus Statement) ของ A.S.P.E.N., ESPEN, และ Academy(6-10) เป็นแบบฟอร์ม NT 2013 ต่อมาในปี พ.ศ. 2561 สมาคมผู้ให้อาหารทางหลอดเลือดดำและทางเดินอาหารแห่ง ประเทศไทย (Society of Parenteral and Enteral Nutrition of Thailand : SPENT) ได้รับการสนับสนุนจาก กระทรวงสาธารณสุข จัดกิจกรรมการพัฒนาโรงพยาบาลคุณภาพโภชนบำบัด ส่งผลให้โรงพยาบาลต่าง ๆ มีการประเมิน-คัดกรอง และจัดตั้งทีมโภชนบำบัดเพิ่มขึ้น เป็นการเพิ่มคุณภาพการดูแลรักษาเชิงรุกด้านโภชนบำบัดให้แก่ผู้ป่วย นอกจากนี้ก็ได้รับการ สนับสนุนจากสำนักงานหลักประกันสุขภาพแห่งชาติ (สปสช.)(11) ให้มีการจ่ายค่าชดเชยค่ารักษาพยาบาลทางการแพทย์เพิ่มขึ้น (reimburstment) โดยเมื่อผลการประเมิน-คัดกรองจาก NT 2013 พบว่าเป็นผู้ป่วยที่มีภาวะทุพโภชนาการ และมีการกำหนด หลักเกณฑ์อย่างเป็นรูปธรรม เป็นการเพิ่มรายได้ให้แก่โรงพยาบาลควบคู่กันไปและหน่วยงานวิจัย HITAP ภายใต้สำนักนโยบาย และยุทธ์ศาสตร์ กระทรวงสาธารณสุข (Health Intervention and Technology Assessment Program)(12) ก็ได้แนะนำ แบบฟอร์ม NT 2013 ในการใช้ประเมิน-คัดกรองภาวะทุพโภชนาการ
