ปัจจัยที่สัมพันธ์กับการเสียชีวิตในโรงพยาบาลและการส่งต่อผู้ป่วยในผู้ป่วยโรคปอดอักเสบ: การวิเคราะห์แบบ Competing-Risks Regression ที่โรงพยาบาลบางคล้า

บทคัดย่อ

ความเป็นมา: โรคปอดอักเสบเป็นสาเหตุสำคัญของการเข้ารับการรักษาในโรงพยาบาลและการเสียชีวิต โดยเฉพาะในผู้สูงอายุและผู้ที่มีโรคร่วม อย่างไรก็ตาม ข้อมูลเกี่ยวกับปัจจัยที่สัมพันธ์กับการเสียชีวิต และการส่งต่อผู้ป่วยในบริบทของโรงพยาบาลชุมชนยังมีจำกัด การศึกษานี้มีวัตถุประสงค์เพื่อประเมินปัจจัยที่สัมพันธ์กับการเสียชีวิตในโรงพยาบาล และการส่งต่อผู้ป่วยไปยังโรงพยาบาลอื่นในผู้ป่วยโรคปอดอักเสบที่เข้ารับการรักษาในโรงพยาบาลชุมชนแห่งหนึ่งของประเทศไทย โดยใช้แบบจำลอง Fine-Gray competing-risks regression วิธีการศึกษา: การศึกษานี้เป็น retrospective cohort study โดยวิเคราะห์ข้อมูลผู้ป่วยอายุ ≥18 ปี ที่ได้รับการวินิจฉัยโรคปอดอักเสบและเข้ารับการรักษาในโรงพยาบาลบางคล้า ระหว่างปี 2015-2025 ข้อมูลถูกสกัดจากเวชระเบียนและรหัสการวินิจฉัยตามระบบ ICD ตัวแปรที่ศึกษา ได้แก่ ข้อมูลประชากร ดัชนี มวลกาย โรคร่วมตาม Charlson comorbidity group และอุณหภูมิร่างกายแรกรับ ผลลัพธ์หลักคือการเสียชีวิตในโรงพยาบาล และผลลัพธ์รองคือการส่งต่อผู้ป่วยไปยังโรงพยาบาลอื่น การวิเคราะห์ใช้แบบจำลอง Fine-Gray competing-risks regression โดยกำหนดค่า p <0.05 เป็นระดับนัยสำคัญทางสถิติ ผลการศึกษา: มีผู้ป่วยโรคปอดอักเสบทั้งหมด 1,816 ราย โดยมีข้อมูลผลลัพธ์สมบูรณ์ 1,815 ราย อายุเฉลี่ย 50.53 ± 18.42 ปี และ 430 ราย (23.68%) มีอายุ ≥65 ปี ผู้ป่วยส่วนใหญ่จำหน่ายกลับบ้าน 1,712 ราย (94.33%) ขณะที่เสียชีวิตในโรงพยาบาล 30 ราย (1.65%) และถูกส่งต่อ 73 ราย (4.02%) ในแบบจำลองหลายตัวแปร อายุ ≥65 ปีสัมพันธ์กับการเสียชีวิตในโรงพยาบาลสูงขึ้นอย่างมีนัยสำคัญ (adjusted SHR 2.59, 95% CI 1.06-6.34; p = 0.038) เช่นเดียวกับ BMI <18.5 kg/m² (adjusted SHR 4.72, 95% CI 1.41- 15.82; p = 0.012) และการมีโรคร่วม ≥2 โรค (adjusted SHR 6.20, 95% CI 2.37-16.25; p <0.001) อุณหภูมิร่างกาย <36.7°C และ >37.8°C ไม่สัมพันธ์กับการเสียชีวิตหลังการปรับค่าส่วนการวิเคราะห์การส่ง ต่อพบว่าอายุ ≥65 ปีเป็นปัจจัยเดียวที่สัมพันธ์อย่างมีนัยสำคัญ (adjusted SHR 2.64, 95% CI 1.53-4.56; p <0.001) สรุป: ในผู้ป่วยโรคปอดอักเสบที่เข้ารับการรักษาในโรงพยาบาลชุมชนแห่งนี้ อายุ ≥65 ปี ภาวะ น้ำหนักต่ำกว่าเกณฑ์ และการมีโรคร่วมหลายโรคสัมพันธ์กับการเสียชีวิตในโรงพยาบาล ขณะที่อายุ ≥65 ปี สัมพันธ์กับการส่งต่อผู้ป่วย ผลการศึกษานี้สนับสนุนการประเมินความเสี่ยงตั้งแต่แรกรับเพื่อช่วยวางแผนการ เฝ้าระวัง การรักษา และการส่งต่อในโรงพยาบาลชุมชน ทั้งนี้ การขาดข้อมูล BMI และอุณหภูมิในสัดส่วนสูง เป็นข้อจำกัดสำคัญที่ควรพิจารณา
