ปัจจัยที่มีความสัมพันธ์ต่อการเกิดโรคหลอดเลือดสมองตีบในผู้ป่วยที่เข้ารับการรักษาในโรงพยาบาลบัวใหญ่

บทคัดย่อ

บทนำ: โรคหลอดเลือดสมองเป็นสาเหตุสำคัญของความพิการและการเสียชีวิตของประชากรทั่วโลก วัตถุประสงค์เพื่อศึกษาปัจจัยที่มีความสัมพันธ์กับการเกิดโรคหลอดเลือดสมองตีบในผู้ป่วยที่เข้ารับการรักษาในโรงพยาบาลบัวใหญ่ จังหวัดนครราชสีมา วัสดุและวิธีการ: การวิจัยเป็นรูปแบบ Retrospective Case Control Study กลุ่มตัวอย่างคือเวชระเบียนผู้ป่วยที่ได้รับการวินิจฉัยโรคหลอดเลือดสมองตีบ และกลุ่มควบคุมคือเวชระเบียนผู้ป่วยที่ไม่ใช่โรคหลอดเลือดสมองตีบ ทั้งเพศชายและเพศหญิงอายุ 18 ปีขึ้นไป เข้ารับการรักษาระหว่างวันที่ 1 ตุลาคม พ.ศ. 2567 ถึง 30 กันยายน พ.ศ. 2568 จำนวนกลุ่มละ 666 ราย เก็บรวบรวมข้อมูลจากโปรแกรม HOSxP วิเคราะห์ข้อมูลทั่วไปด้วยสถิติเชิงพรรณนา และวิเคราะห์ปัจจัยที่มีความสัมพันธ์กับการเกิดโรคหลอดเลือดสมองตีบด้วยสถิติ Multiple logistic regression analysis ผลการวิจัย: พบว่าปัจจัยหลักที่มีความสัมพันธ์กับการเกิดโรคหลอดเลือดสมองตีบอย่างมีนัยสำคัญทางสถิติ (p< 0.001) ได้แก่ โรคความดันโลหิตสูง (OR = 3.55, 95%CI: 2.69-4.69), โรคเบาหวาน (OR = 2, 95% CI: 1.53-2.61), ภาวะหัวใจห้องบนสั่นพลิ้ว (Atrial fibrillation) (OR = 1.38, 95%CI: 1.09-3.25), ไขมันในเลือดสูง (OR =1.73, 95%CI: 1.19-2.02) และการสูบบุหรี่ (OR = 1.12, 95%CI: 1.05-1.25) ปัจจัยรองที่มีความสัมพันธ์กับการเกิดโรคหลอดเลือดสมองตีบอย่างมีนัยสำคัญทางสถิติ ได้แก่ อายุ 61–80 ปี (OR = 1.99, 95%CI: 1.29–3.06, p = 0.020) และการดื่มสุรา (OR = 1.22, 95%CI: 1.12–1.38, p < 0.001) สรุปผล: การป้องกันและควบคุมโรคเรื้อรัง ได้แก่ โรคความดันโลหิตสูง โรคเบาหวาน ไขมันในเลือดสูงหัวใจเต้นผิดจังหวะ การไม่สูบบุหรี่ และไม่ดื่มสุรา ป้องกันการเกิดโรคหลอดเลือดสมองตีบได้
