ภาวะ HELLP Syndrome : บทบาทพยาบาลเพื่อการป้องกันและดูแลรักษา

บทคัดย่อ
ภาวะ HELLP Syndrome คือกลุ่มอาการของเม็ดเลือดแดงแตก (Hemolysis) เอนไซม์ตับเพิ่มขึ้น (Elevated Liver Enzyme levels) และภาวะเกล็ดเลือดต่ำ (Low platelet count) ซึ่งถือเป็นภาวะที่มีความรุนแรงในหญิงตั้งครรภ์ภาวะครรภ์เป็นพิษชนิดรุนแรง (Preeclamsia with severe feature) ในประเทศไทยมีรายงานอัตราการเกิด HELLP Syndrome ร้อยละ4.1-12.5 ของหญิงตั้งครรภ์ที่มีภาวะความดันโลหิตสูงชนิดรุนแรง ซึ่งภาวะนี้นำไปสู่ภาวะทุพลภาพและการเสียชีวิตในมารดาและทารก ดังนั้น การดูแลป้องกันหรือการวินิจฉัยที่รวดเร็ว นำไปสู่การรักษาที่ทันท่วงที จะช่วยลดภาวะวะแทรกซ้อนและอัตราการเสียชีวิตในมารดาและทารก พยาบาลมีบทบาทสำคัญในการให้การพยาบาลเพื่อป้องกันและดูแลรักษาแก่หญิงตั้งครรภ์ที่มีภาวะ HELLP Syndrome ทั้งการดูแลให้นอนพักผ่อนและติดตามความดันโลหิตอย่างและอัตราการเต้นของหัวใจทารกอย่างต่อเนื่อง ดูแลให้ได้รับยาป้องกันการชักและยาควบคุมความดันโลหิต หรือดูแลให้ได้รับส่วนประกอบของลือดตามแผนการรักษาและการติดตามผลข้างเคียง รวมถึงประสานทีมสหสาขาวิชาชีพเพื่อร่วมกันวางแผนการสิ้นสุดการตั้งครรภ์ และติดตามดูแลในระยะหลังคลอดเพื่อเฝ้าระวังภาวะชักจากภาวะความดันโลหิตสูงและการตกเลือดหลังคลอด
