Developing Model for Continuing Palliative Care at Home in Contracting Unit for Primary Care

Abstract

This research and development aims to, 1) analyze situations and problems in continuation of palliative care at home; 2) developing model for continuing palliative care at home; and 3) evaluate a model of continuation palliative care at home in the district contracting unit for primary care in rural area. The research was divided into 3 phases. A total of 73 participants were involved in the study. Research tools are an in-depth interview guideline, group discussion questions guideline, participant observation form, comment form on the suitability and feasibility of practice satisfaction assessment form and satisfaction assessment form. Analyze quality data with thematic analysis, quantitative data were analyzed with descriptive statistics and Paired Sample t-test. The results showed that 1) Terminally ill patients in primary health care have limited access to continuing palliative care at home and receive palliative care services at homes that are ineffective as a result, patients receive not coverage services and do not meet the standards of care. 2) The developed continuing palliative care at home model consisted of 3 steps: 1st preparation for palliative care, 2nd palliative care at home and 3rd follow through and evaluation. The resulting model had an average of 98.21% average suitability, 95.25 % average feasibility of implementation. 3) The model was used to experiment with palliative patients in the research area had a significantly higher satisfaction P<.05. Moreover, the rate of continuing palliative care at home accessibility increased from 51.39% to 81.32%.
