การพยาบาลผู้ป่วยโรคหลอดเลือดสมอง

บทคัดย่อ
การศึกษาครั้งนี้เป็นการศึกษารายกรณีในผู้ป่วยโรคหลอดเลือดสมอง โดยมีวัตถุประสงค์เพื่อหาแนว ทางในการดูแลผู้ป่วยแบบองค์รวมและพัฒนาแผนการจำหน่ายผู้ป่วยโรคหลอดเลือดสมองในหอผู้ป่วย พิเศษอายุรกรรม กลุ่มการพยาบาล โรงพยาบาลเจ้าพระยายมราช ที่มีภาวะพร่องสุขวิทยาส่วนบุคคลญาติ ขาดความรู้ในการดูแลผู้ป่วยคะแนน Bathel ADL Index น้อยกว่า 10 ที่นอนพักรักษาตัวในตึกพิเศษอายุรกรรม ในเดือนสิงหาคม 2553 โดยใช้กระบวนการพยาบาลเป็นหลักแนวคิดในการให้การดูแลผู้ป่วยอย่างครอบคลุม
ผลที่ได้จากการศึกษาพบข้อวินิจฉัยทางการพยาบาล 8 ข้อ ดังนี้ 1) มีภาวะพร่องสุขวิทยาส่วนบุคคลเนื่องจาก ไม่สามารถปฏิบัติกิจวัตรประจำวันได้ 2) มีโอกาสเกิด ภาวะแทรกซ้อน ข้อติดกล้ามเนื้อลีบจากภาวะกล้ามเนื้อ แขนขาขวาอ่อนแรง 3) ผู้ป่วยมีความวิตกกังวล เกี่ยวกับความเจ็บป่วยเรื้อรัง 4) เสี่ยงต่อการเกิดภาวะผิวหนัง เสียหน้าที่ เนื่องจากการกดทับเป็นเวลานาน 5) เสี่ยงต่อการเกิดภาวะร่างกายได้รับสารอาหารไม่เพียงพอ เนื่องจากกล้ามเนื้อที่เกี่ยวข้องกับการกลืนอ่อน แรงและ reflex การกลืนลดลง 6) เสี่ยงต่อการขาดประสิทธิภาพในการจัดการตนเองตามข้อกำหนด ในการรักษา เนื่องจากขาดความรู้ในการปฏิบัติตัวเมื่อ
กลับบ้าน 7) เสี่ยงต่อการได้รับยาไม่ถูกต้อง และมีภาวะแทรกซ้อนจากการใช้ยา เนื่องจากขาดความรู้ 8) การขับถ่ายอุจจาระเปลี่ยนแปลง : ท้องผูก / อุจจาระราด จากข้อวินิจฉัยทางการพยาบาลนำสู่การวางแผนการพยาบาลการปฏิบัติการพยาบาลและ การประเมินผลการพยาบาลที่สอดคล้องกับปัญหาและผู้ป่วยได้รับการวางแผนจำหน่ายในรูปแบบ METHOD และติดตามเยี่ยมทางโทรศัพท์
ข้อเสนอแนะในการศึกษาครั้งนี้ ทีมสุขภาพควรมีการปรับปรุงทีมการดูแลให้เป็นรูปแบบสหสาขา วิชาชีพ มีการติดตามผู้ป่วยหลังจำหน่ายอย่างต่อเนื่อง และมีระบบให้คำปรึกษาเมื่อผู้ดูแลพบปัญหา ในการดูแลผู้ป่วย ด้านผู้ป่วยและญาติควรปฏิบัติตัวตามคำแนะนำอย่างเคร่งครัด

Case study : Stroke

Abstract
The purpose of this study aimed to study individual case study of stroke patients. Objective of the study was to develop holistic nursing care for stroke patients admitted in special medical department, Nursing Department, Chaoprayayommaraj Hospital. Select case study criteria : patient with Impaired personal hygiene, caregiver lack of knowledge, Bathel ADL Index score < 10 , admission in August 2010 .The nursing care for this group of patients was based on nursing process for holistic care giving to the patients. Further to the study, we had found 8 nursing diagnosis namely 1) Impaired personal hygiene related to inability to perform activities of daily living.2) Possibly having complications, congested joints, and leaned muscle due to weaken limb muscles.3) Anxiety related to chronic diseases.4) Impaired skin integrity related to immobility. 5) Imbalance nutrition related to insufficient intake due to weakened swallow related muscle and decreased swallowing reflex. 6) Risk for ineffective therapeutic Regiment management related to altered ability to maintain self at home. 7) Risk of improper medicine taking and side effect of taking medicine related to inadequate knowledge. 8) Total incontinence related to sphincter control: constipation and diarrhea. Nursing diagnoses brought about nursing care plan, nursing implementation and nursing evaluation complied with problems. Patients were released discharge planning, METHOD and follow up on telephones.

The result of this study suggested that health care team should be improved as
multidiscipline team work, there should be continuously follow up after discharge, providing
consult and advice on how to take care patients for the caregivers, and education for patients and their relatives how to follow the instructions properly.
