Development of Pharmaceutical Care Model for Patient Care in the Warfarin Clinic at Khongchiam Hospital, Khongchiam District, Ubon Ratchathani Province

Abstract

This action research aimed to develop the pharmaceutical care model for patient care in the warfarin clinic at Khongchiam Hospital, Khongchiam District, Ubon Ratchathani Province. This study included 9 participants and 30 warfarin-treated patients. Duration of study between July – December 2022. The quantitative research tools were questionnaires, and the qualitative research tools were focus groups that met in person. Quantitative data were analyzed by descriptive statistics (frequency, percentage, mean, standard deviation) and inferential statistics (Wilcoxon matched-pairs singed-ranks test). Qualitative data were analyzed by content analysis.
The results showed that development processes comprised 8 stages 1) meeting with participants 2) study the context and problem 3) problem selection and planning 4) pharmaceutical care implements  5) educating the patient 6) observe and record 7) evaluation  8) lesson learned activities. The developed model was the Policy, Clinical Practice Guideline, Equipment (PCE) model. That was highlighted to implement of the policy and have clear responsibilities, the clinical practice guideline, including the support of equipment. This is an excellent result, resulting in successful and efficient operations.
This study demonstrated an improvement in drug related problems, knowledge, practice, clinical outcome and satisfaction with the statistical significance (p-value < 0.05).
