Abstract
งานประจำที่ซ้ำๆจำเจ หากเราเห็นปัญหาและพยายามแก้ไขปัญหาด้วยการใช้หลักฐานเชิงประจักษ์และความร่วมแรงร่วมใจของทีม จะเปลี่ยนปัญหาให้กลายเป็นความสนุกท้าทายและความภูมิใจเมื่อความปลอดภัยเกิดขึ้นแก่ผู้ป่วย เรื่องเล่านี้เป็นตัวอย่างความสำเร็จที่เกิดขึ้นจากต่อม “เอ๊ะ” การสังเกตปัญหา การแสดงความคิดเห็นของผู้ปฏิบัติงาน การรับฟังของหัวหน้า การสื่อสารระหว่างทีม การสร้างแรงจูงใจด้วยการทำให้ทีมเห็นความสำคัญของปัญหาก่อนลงมือปฏิบัติ ความช่วยเหลือแบ่งปันหน้าที่ในทีมทุกคน การกำกับติดตามดูแล และการประเมินผลลัพธ์ ส่งผลลดอัตราการเกิดภาวะเยื่อบุช่องปากอักเสบ (mucositis) ในผู้ป่วยมะเร็งศีรษะและคอที่ได้รับการฉายแสงได้จริง
