Nurses’s Role in Caring for Patients with Heart Failure at Ambulatory Care

Abstract
Heart Failure (HF) is a clinical syndrome caused by structural or functional abnormalities. As a result, the heart is unable to pump blood to the tissues or receives blood return to the heart normally. The patients who admit and discharge from the hospital will become the patients with chronic heart failure. Inadequate self-care can cause hospital re-admission. The effective self-care consists of educating patients on self-care maintenance, self-care monitoring and self-care management. Nurses should encourage the patients to gain confidence and maintain correct and consistent self-care. Therefore, nurses who work at ambulatory care have the crucial role in caring for patients with chronic heart failure. These patients need continuing care after discharge from the hospital. The goal is for patients to receive the treatment for heart failure continuously, and have quality of life.
