บทคัดย่อ

การผ่าตัดแบบวันเดียวกลับ เป็นการรักษาที่นิยมเพิ่มขึ้นในระบบสาธารณสุขทั่วโลก เนื่องจากลด
ระยะเวลานอนโรงพยาบาล ลดค่าใช้จ่าย การวัดความสำเร็จของผ่าตัด ประเมินจากการที่ผู้ป่วย
กลับบ้านได้ตามแผนโดยไม่มีภาวะแทรกซ้อน หรือต้องนอนโรงพยาบาลโดยไม่วางแผน
การศึกษาย้อนหลังเชิงพรรณนาเพื่อศึกษาปัจจัยที่มีความสัมพันธ์กับความสำเร็จของการผ่าตัด
แบบวันเดียวกลับ ในผู้ป่วยที่เข้ารับการผ่าตัดแบบวันเดียวกลับที่โรงพยาบาลรามาธิบดีจักรี
นฤบดินทร์ ตั้งแต่วันที่ 1 มกราคม พ.ศ. 2565 ถึง 30 กันยายน พ.ศ. 2567 จำนวน 178 ราย
เก็บข้อมูลจากเวชระเบียน เครื่องมือวิจัยประกอบด้วย 1) แบบบันทึกข้อมูลทั่วไป 2) แบบ
ประเมิน ASA classification และ 3) แบบประเมินการตรวจวิเคราะห์ก่อนระงับความรู้สึก
และผ่าตัด ใช้สถิติเชิงพรรณนา ได้แก่ ค่าความถี่ ค่าร้อยละ ค่าเฉลี่ยส่วนเบี่ยงเบนมาตรฐาน
การทดสอบไคสแควร์ และสถิติฟิชเชอร์ เพื่ออธิบายความสัมพันธ์ ผลการศึกษาพบความสำเร็จ
ของการผ่าตัดแบบวันเดียวกลับ 150 ราย (ร้อยละ 84.27) ปัจจัยที่มีความสัมพันธ์กับ
ความสำเร็จในผู้ป่วยกลุ่มนี้ได้แก่ การได้รับยาแก้ปวดแบบผสมผสาน (p = .008) การได้รับยา
หย่อนกล้ามเนื้อ (p = .009)การได้รับยาป้องกันภาวะคลื่นไส้อาเจียนหลังผ่าตัด (p = .015)
ชนิดการระงับความรู้สึก (p < .001) และการประเมิน ASA classification (p = .023)
ผลของการวิจัยนี้เป็นประโยชน์ในการเฝ้าระวัง และวางแผนในการดูแลผู้ป่วยในกลุ่มนี้ต่อไป
ผลการศึกษานี้ พบว่า ความสำคัญของแนวทางการจัดการดูแลผู้ป่วยในระยะผ่าตัด โดยการนำ
กลยุทธ์ที่มุ่งเน้นการควบคุมอาการปวด และการป้องกันอาการคลื่นไส้อาเจียนหลังผ่าตัด เพื่อ
เพิ่มอัตราความสำเร็จของการผ่าตัดแบบวันเดียวกลับ และการใช้ทรัพยากรทางการแพทย์
อย่างมีประสิทธิภาพ
