บทคัดย่อ

การศึกษานี้เป็นการศึกษาแบบย้อนหลังเพื่อเปรียบเทียบผลลัพธ์ทางคลินิกของการสวนหัวใจ
ระหว่างการเข้าทางหลอดเลือดแดงเรเดียล และการเข้าทางหลอดเลือดแดงเฟมอรัล ในผู้ป่วยที่
เข้ารับการสวนหัวใจระหว่างเดือนมกราคมถึงธันวาคม พ.ศ. 2567 โดยทบทวนเวชระเบียน
ผู้ป่วย 109 ราย (กลุ่มเข้าทางเรเดียล 38 ราย และ กลุ่มเข้าทางเฟมอรัล 71 ราย) ตัวแปร
ต่อเนื่องวิเคราะห์ด้วยการทดสอบแมนน์–วิทนีย์ ยู และตัวแปรจัดกลุ่มวิเคราะห์ ด้วยการทดสอบ
ฟิชเชอร์แบบเอ็กแซกต์ กำหนดระดับนัยสำคัญที่ p < .05
ผลการศึกษา พบว่า กลุ่มการเข้าทางหลอดเลือดแดงเฟมอรัล มีอายุเฉลี่ยสูงกว่ากลุ่มการเข้าทาง
หลอดเลือดแดงเรเดียล อย่างมีนัยสำคัญทางสถิติ (p < .001) และมีสัดส่วนผู้ป่วยที่เคยได้รับ
การสวนหัวใจมาก่อนสูงกว่า (p = .024) สำหรับผลลัพธ์หลัก ไม่พบความแตกต่างของภาวะ
แทรกซ้อนจากตำแหน่งเข้าถึงหลอดเลือดระหว่างสองกลุ่ม (ร้อยละ 2.6 เทียบกับร้อยละ 2.8;
p = 1.000) และไม่พบภาวะแทรกซ้อนรุนแรง สำหรับผลลัพธ์รอง กลุ่มการเข้าทาง
หลอดเลือดแดงเรเดียล มีระยะเวลาการทำหัตถการสั้นกว่า (p = .030) มีอัตราการจำหน่าย
ภายในวันเดียวกันสูงกว่า (p = 0.001) และมีระยะเวลานอนโรงพยาบาลสั้นกว่า (p = .002)
ขณะที่ปริมาณสารทึบแสงไม่แตกต่างกัน
โดยสรุปการศึกษานี้ไม่พบความแตกต่างของภาวะแทรกซ้อนจากตำแหน่งเข้าถึงหลอดเลือด
ระหว่างการเข้าทางหลอดเลือดแดงเรเดียล และการเข้าทางหลอดเลือดแดงเฟมอรัล อย่างไร
ก็ตาม การเข้าทางหลอดเลือดแดงเรเดียล มีข้อได้เปรียบด้านประสิทธิภาพและการลดระยะเวลา
นอนโรงพยาบาล ผลการศึกษานี้สนับสนุนให้พิจารณา การเข้าทางหลอดเลือดแดงเรเดียล
เป็นทางเลือกที่ควรให้ความสำคัญในผู้ป่วยที่เหมาะสม
