ความสัมพันธ์ของภาวะมวลกล้ามเนื้อน้อยและการเกิดกระดูกสะโพกหักในผู้สูงอายุ

ภูมิหลัง : ภาวะมวลกล้ามเนื้อน้อย หมายถึง การลดลงของ มวลกล้ามเนื้อและกำลังของกล้ามเนื้อ ซึ่งอาจสูงผลต่อการหกล้ม และการเกิดกระดูกหักได้ในผู้สูงอายุ เป็นเหตุให้ทุพพลภาพและเสีย ชีวิตได้
วัตถุประสงค์ : ศึกษาหาความชุกและความสัมพันธ์ของภาวะ มวลกล้ามเนื้อน้อยต่อการเกิดกระดูกสะโพกหักในผู้สูงอายุ
วิธีการ : การวิจัยเชิงวิเคราะห์ ณ จุดเวลาใด เวลาหนึ่ง ในการประเมินภาวะ มวลกล้ามเนื้อน้อย โดยวัดค่ามวลกล้ามเนื้อ (lean tissue index: LTI) ด้วยการใช้เครื่องวัดองค์ประกอบในร่างกาย (body composition monitor: BCM) ในผู้ป่วยสูงอายุที่มีปัญหากระดูก สะโพกหัก เปรียบเทียบกับผู้สูงอายุที่ไม่มีปัญหากระดูกหัก
ผล : ผู้เข้าร่วมการศึกษาทั้งหมด 98 ราย เป็นกลุ่มที่มีกระดูกสะโพกหัก 48 ราย (ชายร้อยละ 25 หญิงร้อยละ 75) และกลุ่มที่ไม่มีกระดูกหัก 50 ราย (ชายร้อยละ 40 หญิงร้อยละ 60) พบผู้ป่วย 9 รายในกลุ่ม ที่มีกระดูกหัก มีภาวะมวลกล้ามเนื้อน้อย แตกต่างจากกลุ่มที่ไม่มี กระดูกหัก ซึ่งไม่พบผู้ที่มีภาวะมวลกล้ามเนื้อน้อย (p=0.001) ใน ผู้ป่วยสูงอายุที่มีปัญหากระดูกสะโพกหักมีภาวะมวลกล้ามเนื้อเฉลี่ย น้อยกว่าผู้สูงอายุที่ไม่มีปัญหากระดูกหักอย่างมีนัยสำคัญทางสถิติ (LTI 6.89 Kg/m2 vs 7.43 Kg/m2 , p=0.013) โดยเฉพาะอย่างยิ่ง เพศหญิงที่กระดูกหักมีมวลกล้ามเนื้อเฉลี่ยน้อยกว่าหญิงที่ไม่มีปัญหา กระดูกหัก อย่างมีนัยสำคัญทางสถิติ (LTI 6.37 Kg/m2 vs 6.81 Kg/m2 , p=0.001)
สรุป : ภาวะมวลกล้ามเนื้อน้อย ที่พบมากในผู้สูงอายุ ส่งผล ต่อการหกล้มและกระดูกสะโพกหัก โดยเฉพาะอย่างยิ่งเพศหญิง การวัดมวลกล้ามเนื้อด้วยเครื่อง BCM ซึ่งสามารถใช้ได้ง่าย ราคาไม่ แพง สามารถวินิจฉัยภาวะมวลกล้ามเนื้อน้อยและอาจใช้ทำนายการ หกล้มและการเกิดกระดูกหักของผู้สูงอายุได้ในอนาคต
