อัตราการครอบครองยาต้านการชักของผู้ป่วยโรคลมชักเด็กในสถาบัน สุขภาพเด็กแห่งชาติมหาราชินี

ภูมิหลัง : เด็กโรคลมชักมีความจำเป็นต้องใช้ยาต่อเนื่อง แต่ พบว่ายังมีเด็กโรคลมชักที่ไม่ได้รับยาต้านชักต่อเนื่อง ซึ่งมีสาเหตุเกิด จากยาหมดก่อนมาตรวจตามนัด หรือสิทธิทางการรักษาของผู้ป่วย ทำให้เกิดข้อจำกัดของการได้รับยา คือการรับยากันชักในผู้ป่วยสิทธิบัตรทองแต่ละโรงพยาบาลมีข้อจำกัด ส่งผลให้ผู้ป่วยได้รับยาจำนวน จำกัด แต่ยังไม่มีการศึกษาเรื่องอัตราการครอบครองยา (medicine possession ratio, MPR) ซึ่งเป็นตัวชี้วัดที่สามารถบอกได้ว่า ผู้ป่วยได้รับยามากน้อยเพียงพอต่อการรักษาหรือไม่
วัตถุประสงค์ : เพื่อประมาณการอัตราการครอบครองยาต้านการชักโดยเปรียบ เทียบระหว่างสิทธิการรักษาพยาบาล
วิธีการ : การศึกษานี้เป็นการ วิเคราะห์ข้อมูลแบบย้อนหลัง และหาปัจจัยที่มีความสัมพันธ์กับ ความผันแปรของอัตราการครอบครองยา ผู้ป่วยที่ศึกษาคือเด็กโรค ลมชักที่มาใช้บริการและได้รับการสั่งใช้ยาตั้งแต่ 1 มกราคม พ.ศ. 2555 ถึง 31 ธันวาคม พ.ศ. 2560 (72 เดือน) วิเคราะห์ข้อมูลโดย คานวณค่าเฉลี่ยของอัตราการครอบครองยาสาหรับแต่ละสิทธิการ รักษา และวิเคราะห์ปัจจัยที่สัมพันธ์กับอัตราการครอบครองยาโดย ใช้สมการถดถอยเชิงเส้นตรง (linear regression)
ผล : โดยเฉลี่ย อัตราการครอบครองยาสาหรับยาต้านการชักโดยรวมในช่วงเวลา 6 ปีที่ศึกษาคิดเป็นร้อยละ 63.3 (SD,29.9%) เด็กโรคลมชักสิทธิ บัตรพิการ และเด็กโรคลมชักที่มีการเปลี่ยนสิทธิจากบัตรทองเป็น บัตรผู้พิการ มีอัตราการครอบครองยาต้านชักโดยเฉลี่ยสูงกว่าสิทธิ การรักษาอื่น ผลการวิเคราะห์ปัจจัยที่ความเกี่ยวข้องกับอัตราการ ครอบครองยาต้านชักด้วยสมการถดถอยเชิงเส้น พบว่า เด็กโรคลม ชักสิทธิบัตรพิการมีอัตราการครอบครองยาต้านชักมากกว่าเด็กโรค ลมชักสิทธิบัตรทองส่งตัวนอกเขตรับยาด้วยตนเองโดยเฉลี่ย 7.0 จุด (%point) (p<0.05) และเด็กโรคลมชักที่มารับยามากกว่า 5 ปี อัตรา การครอบครองยาต้านชักมากกว่าเด็กโรคลมชักที่มารับยาน้อยกว่า หรือเท่ากับ 1 ปีโดยเฉลี่ย 27.1 (%point) (p<0.05) ขณะเดียวกัน ปัจจัยเรื่องภูมิลำเนาไม่มีความเกี่ยวข้องกับอัตราการครอบครอง ยาต้านชัก (p=0.766)
สรุป : เด็กโรคลมชักแต่ละสิทธิการรักษามี อัตราครอบครองยาต้านชักแตกต่างกัน ขึ้นกับสิทธิการรักษาและ ระยะเวลาในการใช้ยาเป็นสำคัญ
