บทคัดย่อ

ภูมิหลัง: ยากลุ่ม opioid ร่วมกับ benzodiazepine ใช้กันอย่างแพร่หลายในหอผู้ป่วยเด็กวิกฤต โดยแพทย์เป็น ผู้สั่งใช้ยาเพื่อลดความเจ็บปวดจากตัวโรคและหัตถการและ ช่วยให้ผู้ป่วยสามารถหายใจเข้ากับเครื่องช่วยหายใจ โดยผู้ป่วย ส่วนใหญ่ใช้ยาในรูปแบบหยดต่อเนื่องทางหลอดเลือดดำ เป็นระยะเวลานาน เมื่อหยุดยาทันทีหรือลดขนาดยาอย่าง รวดเร็ว อาจนำไปสู่อาการภาวะถอนยากลุ่ม opioid หรือ benzodiazepine ได้ ทางสถาบันสุขภาพเด็กฯ ได้พัฒนา แนวทางการป้องกันและรักษาภาวะถอนยาแก้ปวดกลุ่ม opioid และยานอนหลับกลุ่ม benzodiazepine เพื่อนำมาใช้ในหอ ผู้ป่วยเด็กวิกฤตและใช้เครื่องมือ The Withdrawal Assessment Tool Version 1 (WAT-1) ในการประเมินภาวะถอนยา เพื่อให้ทราบอุบัติการณ์การเกิดภาวะถอนยาในผู้ป่วยเด็ก จึงเป็นที่มาในการศึกษาครั้งนี้ วัตถุประสงค์: เพื่อหาอุบัติการณ์ การเกิดภาวะถอนยากลุ่ม opioid และ benzodiazepine และหาขนาดยาสะสมรวมถึงระยะเวลาเฉลี่ยในการได้รับยา fentanyl หรือ morphine ร่วมกับยา midazolam ในกลุ่มที่ เกิดภาวะถอนยา วิธีการ: เป็นการศึกษาเชิงพรรณนาเก็บข้อมูล ณ หอผู้ป่วยเด็กวิกฤต สถาบันสุขภาพเด็กแห่งชาติมหาราชินี ตั้งแต่วันที่ 1 มกราคม 2566 ถึง 10 กรกฎาคม 2567 มีผู้ร่วม การวิจัยจำนวน 64 ราย อายุระหว่าง 2 เดือน ถึง 18 ปี และ ได้ใช้แนวทางการป้องกันและรักษาภาวะถอนยาแก้ปวดกลุ่ม opioid และยานอนหลับกลุ่ม benzodiazepine ผล: ผู้เข้าร่วม การศึกษา 64 ราย มีอายุระหว่าง 2 เดือน ถึง 16 ปี พบ ผู้ป่วยเกิดภาวะถอนยา 19 ราย (ร้อยละ 29.7) เป็นกลุ่มที่ได้ รับยา midazolam ร่วมกับ fentanyl 17 ราย (ร้อยละ 89.5) และกลุ่มที่ได้รับยา midazolam ร่วมกับ morphine 2 ราย (ร้อยละ 10.5) โดยมีขนาดยาสะสมเฉลี่ยของ midazolam fentanyl และ morphine เท่ากับ 28.99, 0.44 และ 5.59 มิลลิกรัมต่อกิโลกรัมตามลำดับ ส่วนระยะเวลาเฉลี่ยในการ ได้รับยา opioid และ benzodiazepine ถึงวันที่ผู้ป่วยเกิด ภาวะถอนยาเท่ากับ 7.74 วัน สรุป: อุบัติการณ์การเกิดภาวะ ถอนยากลุ่ม opioid หรือ benzodiazepine ในรูปแบบหยด ต่อเนื่องทางหลอดเลือดดำ เมื่อประเมินด้วยเครื่องมือ WAT-1 เท่ากับร้อยละ 29.7
