การศึกษาประสิทธิผลของฟันเทียมทั้งปากเปรียบเทียบกับฟันเทียมที่เสริมการยึดด้วยรากฟันเทียมในขากรรไกรล่าง ในจังหวัดขอนแก่น

ภูมิหลัง: การสำรวจสภาวะทันตสุขภาพครั้งที่ 9 ปี พ.ศ. 2566 พบว่าผู้สูงอายุ 60-80 ปี มีจำนวนฟันเฉลี่ย 15.65 ซี่/คน ซึ่งน้อยกว่าจำนวนที่ควรมีกรณีที่ไม่มีฟันทั้ง ปากคิดเป็นร้อยละ 14.65 ซึ่งรักษาโดยการใส่ฟันเทียมทั้งปาก ร้อยละ 10 ของผู้ใส่ฟันเทียมทั้งปากพบปัญหาฟันเทียมล่างขยับ ขณะบดเคี้ยวอาหาร กระทรวงสาธารณสุขตระหนักถึงความ สำคัญของปัญหาดังกล่าว จึงได้จัดทำ "โครงการฟันเทียม รากฟัน เทียม เนื่องในโอกาสมหามงคลเฉลิมพระชนมพรรษา 72 พรรษา 28 กรกฎาคม 2567" วัตถุประสงค์: เพื่อประเมินประสิทธิผลของการใช้รากฟันเทียมนวัตกรรมของไทยเสริมการยึดอยู่ของฟันเทียมทั้งปากล่าง โดยเปรียบเทียบคุณภาพชีวิตมิติสุขภาพช่องปาก ประสิทธิภาพการบดเคี้ยว ความพึงพอใจก่อนและหลังร่วมโครงการฟันเทียม รากฟันเทียมฯ และการรับรู้ ความเจ็บปวด วิธีการ: เป็นการวิจัยกึ่งทดลอง เก็บข้อมูลด้วย แบบสัมภาษณ์ผู้ป่วยที่ร่วมโครงการฟันเทียม รากฟันเทียมฯ จำนวนกลุ่มตัวอย่าง 71 ราย จากสถานบริการ 9 แห่ง ใน จังหวัดขอนแก่น ผล: ค่าคะแนนคุณภาพชีวิตในมิติสุขภาพช่องปาก (OIDP index) มีค่าเฉลี่ยก่อนการรักษาร้อยละ 20.90 คะแนนประสิทธิภาพการบดเคี้ยวมีค่าเฉลี่ยร้อยละ 34.21 และความพึงพอใจมีค่าเฉลี่ยร้อยละ 56.80 ภายหลัง การรักษา มีค่า OIDP index เฉลี่ยร้อยละ 0.59 คะแนน ประสิทธิภาพการบดเคี้ยวเฉลี่ยร้อยละ 97.07 ทั้ง 2 ค่า มีความ ต่างกันอย่างมีนัยสำคัญทางสถิติ (p-value < .001) ค่าเฉลี่ยความพึงพอใจร้อยละ 97 อยู่ในระดับมากที่สุด วัดความเจ็บ ปวด (pain score) ขณะผ่าตัดใส่รากฟันเทียมเปรียบเทียบกับระดับความเจ็บปวดที่ผู้ป่วยเคยถอนฟัน พบว่ามีค่าน้อยกว่า การถอนฟันอย่างมีนัยสำคัญทางสถิติ (p-value < .001) สรุป: การใช้รากฟันเทียมนวัตกรรมของไทยเสริมการยึดอยู่ของฟันเทียม ตามโครงการฟันเทียมรากฟันเทียมฯ นี้ พบค่าประสิทธิภาพการบดเคี้ยวมีค่าคะแนนเฉลี่ยที่เพิ่มขึ้นเป็น 2.84 เท่า และมีค่าคุณภาพชีวิตที่ดีขึ้น
