ผลของการใช้เครื่องมือส่งสัญญาณ ในการค้นหาการเกิดกลุ่มอาการเคลื่อนไหวผิดปกติจากการใช้ยาต้านอาการทางจิตของโรงพยาบาลธัญญารักษ์สงขลา

ภูมิหลัง: กลุ่มอาการเคลื่อนไหวผิดปกติอีพีเอส (extrapyramidal side effects; EPS) เป็นอาการไม่พึงประสงค์จากยาที่พบได้บ่อยจากการใช้ยาต้านอาการทางจิต เครื่องมือส่งสัญญาณเป็นวิธีค้นหาเหตุการณ์ไม่พึงประสงค์ได้มากกว่าการรายงานโดยสมัครใจ ผู้วิจัยจึงมีแนวคิดในการนำเครื่องมือส่งสัญญาณมาใช้ในการค้นหาผู้ป่วยที่เกิด EPS จากยาต้านอาการทางจิตในโรงพยาบาล วัตถุประสงค์: เพื่อศึกษาผลการค้นหาการเกิด EPS ของเครื่องมือส่งสัญญาณแต่ละชนิด และเพื่อศึกษาถึงอุบัติการณ์การเกิด EPS จากยาต้านอาการทางจิต วิธีการ: เป็นการวิจัยย้อนหลัง โดยทบทวนเวชระเบียนประเภทผู้ป่วยในหลังจากผู้ป่วยจำหน่ายออกจากโรงพยาบาล ค้นหาเครื่องมือส่งสัญญาณ 4 ชนิด ได้แก่ การสั่งใช้ยา Benztropine injection การสั่งใช้ยา Diazepam injection การเริ่มยาหรือเพิ่มขนาดยา Trihexyphenidyl oral และการเริ่มยาหรือเพิ่มขนาดยา Propanolol oral เวชระเบียนที่พบการใช้เครื่องมือส่งสัญญาณ จะนำมาสู่การใช้แบบประเมินการเกิดอาการไม่พึงประสงค์จากยาคือ Naranjo’s algorithm หากมีผลตั้งแต่ 5 คะแนนขึ้นไป (อยู่ในระดับน่าจะใช่ หรือใช่แน่) จะสรุปว่าผู้ป่วยมีการเกิด EPS วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนาและสถิติเชิงอนุมาน Fisher’s exact test โดยกำหนดระดับนัยสำคัญทางสถิติที่ .05 ผล: จากการทบทวนเวชระเบียนจำนวน 430 ราย พบผู้ป่วยที่เกิด EPS จากการใช้เครื่องมือส่งสัญญาณจำนวน 26 ราย มากกว่าการรายงานโดยสมัครใจที่พบการเกิด EPS จำนวน 2 ราย การเริ่มยาหรือเพิ่มขนาดยา Trihexyphenidyl oral เป็นเครื่องมือส่งสัญญาณที่นำมาสู่การพบการเกิด EPS มากที่สุด (จำนวน 23 ครั้ง) ส่วนการสั่งใช้ยา Benztropine injection เป็นเครื่องมือส่งสัญญาณที่มีอัตราการพบการเกิด EPS มากที่สุด (ร้อยละ 100) อัตราการเกิด EPS เท่ากับ 2.03 ต่อ 100 ครั้งของการรักษาในโรงพยาบาล อาการของ EPS ที่พบมากที่สุดคือ Acute dystonia (จำนวน 24 ราย) สรุป: การใช้เครื่องมือส่งสัญญาณทั้ง 4 ชนิด สามารถค้นหาการเกิด EPS จากยาต้านอาการทางจิตได้เพิ่มขึ้น
