ภูมิหลัง: การตรวจคัดกรองมะเร็งลำไส้ใหญ่และไส้ตรง (CRC) ในระยะเริ่มต้น เช่น การตรวจหาเลือดแฝงในอุจจาระด้วยวิธีอิมมูโนเคมี (FIT) นำไปสู่การส่งต่อเพื่อเข้ารับการส่องกล้องลำไส้ใหญ่ที่เพิ่มขึ้น ซึ่งอาจก่อให้เกิดความล่าช้า เนื่องจากขีดจำกัดของศักยภาพในการส่องกล้อง วัตถุประสงค์: การศึกษาครั้งนี้มีวัตถุประสงค์เพื่อพัฒนาระบบการให้คะแนน เพื่อทำนายความน่าจะเป็นของการตรวจพบภาวะเนื้องอกลำไส้ใหญ่หรือมะเร็งลำไส้ใหญ่ ในผู้ป่วยที่เข้ารับการส่องกล้องลำไส้ใหญ่ วิธีการ: การศึกษาแบบย้อนหลังนี้วิเคราะห์ข้อมูลจากผู้ป่วย 345 ราย ที่เข้ารับการส่องกล้องลำไส้ใหญ่ ณ โรงพยาบาลปากเกร็ด จังหวัดนนทบุรี ระหว่างเดือนสิงหาคม พ.ศ. 2560 ถึงเดือนธันวาคม พ.ศ. 2567 ผู้ป่วยถูกแบ่งออกเป็น 2 กลุ่ม ได้แก่ normal group และ tumor group ศึกษาปัจจัยอาการแสดงทางคลินิกและผลการตรวจทางห้องปฏิบัติการ ได้แก่ อาการปวดท้อง, การเปลี่ยนแปลงลักษณะการขับถ่าย, การถ่ายอุจจาระเป็นเลือด, น้ำหนักลด, ภาวะซีด, ประวัติมะเร็งลำไส้ใหญ่และไส้ตรงในครอบครัว, ผลการตรวจ FIT เป็นบวก, และระดับ CEA ในเลือดสูง มาวิเคราะห์เพื่อหาความสัมพันธ์กับภาวะเนื้องอกลำไส้ใหญ่หรือมะเร็งลำไส้ใหญ่ ระบบการให้คะแนนประเมินที่จุดตัด ≥ 3 คะแนน ผล: อาการแสดงและผลการตรวจทางห้องปฏิบัติการหลายปัจจัยแสดงความแตกต่างอย่างมีนัยสำคัญทางสถิติระหว่าง normal group และ tumor group ที่จุดตัด ≥ 3 คะแนน ระบบการให้คะแนนที่พัฒนาขึ้นแสดงผล สำหรับเนื้องอกลำไส้ใหญ่ ค่าความไว 78% ความจำเพาะ 56% ค่าทำนายผลบวก 40% และค่าทำนายผลลบ 87% สำหรับมะเร็งลำไส้ใหญ่และไส้ตรง ระบบแสดงค่าความไว 100% ความจำเพาะ 56% ค่าทำนายผลบวก 42% และค่าทำนายผลลบ 100% สรุป: ระบบการให้คะแนนนี้ แสดงให้เห็นถึงประสิทธิผลที่ดีในการระบุผู้ป่วยที่มีความเสี่ยงสูงต่อภาวะเนื้องอกลำไส้ใหญ่หรือมะเร็งลำไส้ใหญ่ จุดตัดที่ 3 คะแนนสามารถตัดผู้ป่วยที่ไม่มีมะเร็งลำไส้ใหญ่และไส้ตรงออกไปได้อย่างมีประสิทธิภาพ เครื่องมือนี้มีศักยภาพในการช่วยจัดลำดับความสำคัญของผู้ป่วยเพื่อเข้ารับการส่องกล้องลำไส้ใหญ่ในสถานพยาบาลที่มีทรัพยากรจำกัดและลดความล่าช้าในการวินิจฉัยในผู้ป่วยที่มีความเสี่ยงสูง
