บทคัดย่อ

ภูมิหลัง: ภาวะภูมิไวเกินเป็นอาการไม่พึงประสงค์ที่อาจเกิดขึ้นอย่างเฉียบพลันจากการได้รับยาเคมีบำบัด โดยเฉพาะกลุ่ม Taxanes เช่น Paclitaxel และ Docetaxel ซึ่งมีรายงานว่าทำให้เกิดอาการตั้งแต่เล็กน้อยจนถึงรุนแรง วัตถุประสงค์: เพื่อศึกษาอุบัติการณ์และระดับความรุนแรงของภาวะภูมิไวเกินในผู้ป่วยมะเร็งที่ได้รับยาเคมีบำบัดกลุ่ม Taxanes วิธีการ: การศึกษานี้เป็นการวิจัยเชิงพรรณนาแบบย้อนหลังจากเวชระเบียนและบันทึกทางการพยาบาล กลุ่มตัวอย่างคือผู้ป่วยมะเร็งจำนวน 120 ราย ที่ได้รับยา Paclitaxel หรือ Docetaxel และเกิดภาวะภูมิไวเกิน ระหว่างวันที่ 1 มกราคม – 31 ธันวาคม พ.ศ. 2567 ใช้แบบประเมิน Common Terminology Criteria for Adverse Events (CTCAE) และวิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนา ผล: ผู้ป่วยส่วนใหญ่เป็นเพศหญิง (90.8%) และอายุ ≥ 41 ปี (82.5%) อาการพบบ่อย ได้แก่ แน่นหน้าอก (77.5%), หน้าแดง (67.5%), หายใจลำบาก (46.7%) มักเกิดใน 16–30 นาทีแรกหลังให้ยา (66.7%) ที่ปริมาณ 11–20 มิลลิลิตร (95.8%) และความเข้มข้น 250–299 มิลลิกรัม (39.2%) ระดับความรุนแรงแตกต่างกัน โดย Docetaxel (n=18) พบ Grade I 61.1%, Grade II 38.9%, ไม่พบ Grade III ขณะที่ Paclitaxel (n=102) พบ Grade I 45.1%, Grade II 53.9%, และ Grade III 1.0% สรุป: ยา Paclitaxel มีแนวโน้มเกิด Grade II–III มากกว่ายา Docetaxel ส่วนยา Docetaxel จะพบ Grade I ในสัดส่วนที่สูงกว่า โดยอาการมักเกิดในรอบที่ 2–3 ของการให้ยา และภายใน 30 นาทีแรกหลังเริ่มให้ยา จึงควรกำหนดช่วงเวลาดังกล่าวเป็นช่วงเฝ้าระวังสำคัญ (critical monitoring window) และพัฒนาระบบคัดกรองก่อนให้ยาสำหรับผู้ป่วยกลุ่มเสี่ยง เพื่อเพิ่มความปลอดภัยในการให้ยาเคมีบำบัด
